A Comparative Study on Allogeneic Bone Graft for the Treatment of Bone Defect after Resection of Benign Bone Tumor in the Long Bone of Extremities

【Objective】To investigate the clinical effects in the use of allogeneic bone transplantation combined with autogeneic bone transplantation after the resection of a benign bone tumor. 【Methods】The patients who received allogeneic bone and autogeneic bone transplantation respectively in our hospital from July 2009 to July 2013 were divided into 2 groups.The period of followup was 12 to 36 months. Based on imaging and clinical results during the followup, the healing speeds and healing rates after bone transplantation between the 2 groups were compared. 【Results】Both initial healing rates of autogeneic bone transplantation and allogeneic bone transplantation were 100%. The mean healing times of autogeneic bone transplantation and allogeneic bone transplantation were (16.4±3.4) weeks and (17.8±3.2) weeks(P>0.05);the mean healing scores were (2.5±0.3) points and (2.4±0.5) points(P>0.05). The mean rates of the postoperative complications of autogeneic bone transplantation and allogeneic bone transplantation were 15.2% and 17.5%(P>0.05). 【Conclusion】Reconstruction using bone autograft and allograft for long bone defects have similar clinical curative effect.

Posterior Debridement and allograft combined with Irrigation and Drainage in the Treatment of Lumbar Intervertebral Infection

【Objective】 To investigate the therapeutic effect of the posterior debridement and interbody fusion combined with irrigation and drainage in the treatment of lumbar intervertebral infection. 【Methods】From January 2010 to June 2013, 28 cases were treated using this method. VAS scores were used to assess pain relief.Erythrocyte sedimentation rate (ESR) and C reactive protein (CRP) were used to assess the general condition.Radiography evaluation was used to assess spinal stability and fusion conditions.【Results】Lower back pain was relieved significantly after 2 to 5 days and disappeared at about two weeks. Postoperative VAS score was significantly lower than the preoperative score. Postoperative body temperature gradually decreased and returned to normal in about 2 to 3 weeks. ESR and CRP levels returned to normal after 2 to 3 weeks. All the patients returned to normal life and were able to work after 6 months. There were no sinus formations or recurrent infections by the oneyear followup. 【Conclusion】 This surgical method is safe, reliable, and effective in the treatment of lumbar intervertebral infection.

Efficacy of Oxycodone on Treatment of Intestinal Angina By Fibercolonscopy

【Objective】To observe the effect of oxycodone injection on the analgesic effect and adverse reaction of outpatients underwent painless fibrecolonoscopy.【Methods】The 120 cases of painless colonoscopy were randomly divided into oxycodone group (Group O) and fentanyl group (Group F). The two groups were treated by intravenous anesthesia with oxycodone combined with propofol and fentanyl combined with propofol respectively. Elements like Preinduction of anesthesia (T0), pre start to check (T1), at the beginning of check (T2), through the hepatic flexure (T3), colonoscopy quit(T4),recovery time(T5), five time points mean arterial pressure (MAP), heart rate (HR), arterial oxygen saturation (SpO2), respiratory rate (RR); the incidence of nausea and vomiting, dizziness, colic and other adverse reactions in the two groups of patients were compared with, and the duration of waking time and the time away from hospital were recorded and checked; The total amount of propofol and the incidence of adverse reactions such as nausea, vomiting, dizziness and colic in the two groups were detected.【Results】All patients were successfully completed colonoscopy. AT time points of T1 and T2, HR, SpO2 and RR in the Group F was significantly lower than those in the Group O (P<0.05). There was no significant difference between the two groups in the examination time, waking time, the time away from the hospital and the total amount of propofol. The probability of postoperative angina occurred in the Group O was obviously less than that of the Group F (P<0.05).【Conclusion】Propofol combined with oxycodone or with fentanyl for painless fibrecolonoscopy are safe and effective, but oxycodone is better with curing intestinal angina and has smaller effect on respiratory, circulatory system , which is more suitable for anesthesia in fibrecolonoscopy.

Screening Process and the Clinical Features Analysis of Primary Aldosteronism

【Objective】To standardize the screening process of patients with primary aldosteronism (PA) to improve the understanding of PA and the diagnostic accuracy of PA. 【Methods】The 204 cases of hypertension patient in our hospital between December 2011 and December 2012 were analyzed retrospectively. We calculated the constituent ratio of PA in all 204 hypertension cases, analyzed the clinical characteristics of these cases including clinical symptoms, laboratory examinations, image examinations, and treatments. Next, we analyzed the difference between the PA patients and the hypertension patients. Afterwards, we calculated the plasm aldosterone to rennin ratio (ARR) and analyzed the specificity sensitivity of different values of ARR and compared the difference of target organ damage between the PA patients and the hypertension patients. Finally, we analyzed the relationship between the length of PA patients and the target organ damage as well as the causes that influence the status of hypertension after the surgery. 【Results】We diagnosed 46 PA patients from hypertension patients by screening of ARR, clinical characteristics, and CT the incidence of PA patients in hypertension was 22.5%. The mean age (45.7±8.9) years; there were male 21 cases and female 25 cases; stage 1 had 3 cases (6.5%), stage 2 had 13 cases (28.3%), stage 3 had 30 cases (65.2%); one of PA patient blood pressure was normal. The average systolic pressure was(178±26.4)mmHg and the average diastolic pressure (111±19.6) was mmHg. We used ARR 20 as the diagnostic cutoff with PAC>15ng/dl, the sensitivity of diagnosis was 85.7%, and the specificity was 84.4%. The PA patients group had more complications than the hypertension patients group. The PA patients group used more antihypertensive drugs than hypertension patients group. 【Conclusion】The incidence of patients with PA was extremely high in stage 2 hypertension, in our study the incidence had reached 20%. ARR have diagnostic value, and with the use of ARR 20 as the diagnostic cutoff with PAC>15ng/dl better sensitivity and specificity can be achieved. The target organs of PA patients were damaged more seriously than hypertension patients. Surgical treatment is good method for PA patients with adrenocotical adenoma; the blood pressure was controlled easily after surgery. Most hypokalemia patients returned to normal. However, if patients had long course, effects would not be good. We advocate early detection, early diagnosis, and early treatment.

Effects of Zinc Alpha 2 Glycoprotein on Maintenance Hemodialysis Quality and Fat Metabolism in Hemodialysis Patients

【Objective】 To explore how maintenance hemodialysis (HD) of patients with zinc (zinc alpha 2glycoprotein alpha 2 glycoprotein, ZAG) affects the level change of metabolism in patients with HD. 【Methods】 Fifty cases undergoing the blood purification treatment of maintenance hemodialysis patients at our hospital were chosen as subjects of study, while clinic examination established 30 healthy cases to serve as the control group. Application of the ELISA detection method was used to detect ZAG, leptin and adiponectin levels, and detection of clinical and biochemical data was used to detect blood urea nitrogen (BUN), creatinine (Cr), fasting blood glucose (FBG), fasting insulin (Fins), total cholesterol (TC), free fatty acid (FFA), high density lipoprotein (HDL) and low density lipoprotein (LDL), and insulin resistance index. Patient sex, age, dialysis duration, and body mass index (BMI) data were also collected. 【Results】 Serum creatinine, BUN, and GFR difference was statistically significant (P< 0.01), but there was no significant difference in FBG, Fins, and HOMRIR between the groups (P>0.05). TG, LDL, FFA, adiponectin, and leptin between the two groups were statistically different (P<0.05), but the difference in TC and HDL was not (P>0.05). ZAG was positively correlated to LP, ADPN, HOMRIR and inversely correlated to CRP, GFR, BMI and HDL. Furthermore, multiple regression analysis showed that ZAG was very closely related with HDL and ADPN.【Conclusion】In maintenance hemodialysis patients, ZAG levels are higher than that in the normal group. ZAG affects lipid and glucose metabolism through LP and ADPN, which in turn reduces BMI and lipid accumulation.

Relationship Among Serum Uric Acid, Hypertension, and Prehypertension in MiddleAged People

【Objective】 To study the relationship among serum uric acid, hypertension, and prehypertension in middleaged people. 【Methods】The 1,613 community residents aged 35~64 years who lived in Shenyang city were investigated. The associations among uric acid, hypertension, and prehypertension were analyzed though questionnaire survey, physical examination and biochemical detection. 【Results】 Means of uric acid levels were (317.32±81.25) μmol/L, (292.35±73.21) μmol/L, and (271.76±63.03) μmol/L in hypertensives, prehypertensives and normotensives respectively, with significant differences among the three groups (P<0.05). The mean level of systolic/diastolic blood pressures were (121.04±17.54)/(73.08±10.63)mmHg, (123.76±19.81)/(75.67±11.75)mmHg, (123.91±18.69)/(75.23±10.61)mmHg, and (131.08±19.54) /(80.02±11.19) mmHg for these with uric acid level in the first, second, third, and fourth quartiles. Adjusted odd ratios of hypertension and prehypertension in quartile 4 of uric acid were 2.31 (95%CI:1.51~3.54, P<0.01) and 1.52 (95%CI:1.03~2.22,P<0.01), with quartile 1 as a reference. 【Conclusion】 Serum uric acid level is associated with both hypertension and prehypertension in middleaged people. Therefore serum uric acid monitoring should be paid attention to.

Clinical Value of Real Time ContrastEnhanced Ultrasound in Diagnosis and Differential of Retroperitoneal Space Occupying Lesions

【Objective】To explore the diagnosis and differential value of real time contrastenhanced ultrasound in retroperitoneal spaceoccupying lesions in patients. 【Methods】A total of 30 cases of retroperitoneal space occupying lesions patients accorded to the pathological results divided into 10 cases of benign lesions (benign group) and 20 cases of malignant lesions (malignant group). The two groups of patients underwent conventional ultrasound and contrastenhanced ultrasound detection, analyzed time intensity curve, and compared check results and pathological results. 【Results】Compared with the benign group, the AS, HT, and API in the malignant group were significantly greater (P<0.05). However, there was no significant difference between the two groups with AT and PT(P>0.05). Ultrasound diagnosis of missed diagnosis rate and the misdiagnosis rate was 40.0% and 20.0%, respectively. The ultrasound contrast time intensity curve rising and descending form parallel detection and missed diagnosis rate and the misdiagnosis rates were 15.0% and 10.0%, respectively. 【Conclusion】Real time contrastenhanced ultrasound can provide a basis for diagnosis and differential of retroperitoneal space occupying benign and malignant lesions, and has certain clinical value.

Clinical Study of Oral S1 with Synchronous Radiotherapy for Locally Advanced Pancreatic Cancer

【Objective】To explore the efficacy and security of oral S1 with synchronous radiotherapy plus chemotherapy for locally advanced pancreatic cancer. 【Methods】: 28 patients with locally advanced pancreatic cancer confirmed by pathology or in accordance with the criteria of clinical diagnosis were collected. Radiotherapy plan was: 10MVX, tumor gross target volume (GTV) were tumor tissues and metastatic lymph nodes, the radiation dose (DT) was 50 Gy/25 times (5 weeks), 1 time / day, plus S1 at a dose of 40~60 mg/d, 1 time / day (according to the patient's body surface area calculation: <1.25 m2, 40 mg/d;≥1.25~1.5 m2, 50 mg/d; ≥1.5 m2, 60 d mg/)during radiotherapy(d1~35); 2 weeks after the end of chemoradiotherapy, the single chemotherapy S1 was administered at a dose of 80~120 mg/d,2 times administered (according to the patient's body surface area calculation: <1.25 m2, 80 mg/d;≥1.25~1.5 m2, 100 mg/d; ≥1.5 m2 120 mg/d), a total of 14 days followed by 1week rest as a treatment cycle. Oral chemotherapy were taken until disease progressed or intolerance to its toxicity occurred. Therapeutic effect evaluation standard was used to evaluate the curative effect of solid tumor response. Toxic and side effects were evaluated by conventional toxicity criteria of the National Cancer Institute ,USA.【Results】Twentysix patients(26/28) finished the whole treatment upto progression .The tumor response rate (CR+PR)was 38.4%(10/26)and tumor control rate (CR+PR+SD)was 80.8%(21/26).The clinical benefit rate was 53.9%(14/26). Followup suggested the median progression free survival and median survival time were 6.3 months and 12.8 months. 1 year overall survival rate was 46.3%.The most common toxic effects were hematologic toxicity (12 degree),the major non hematologic toxicities were nausea and anorexia. 【Conclusion】Oral S1 with synchronous radiotherapy plus chemotherapy was a feasible and effective treatment with mild toxicity for locally advanced pancreatic cancer. The longterm results and toxicity still need further observe.

Clinical Effect of PCNL on Renal Hemodynamics in a Standard Single Channel and a Double Channel

【Objective】To compare the effects of the standard single channel percutaneous nephrolithotomy (PCNL)and double channel percutaneous nephrolithotomy (PCNL) on renal hemodynamics. 【Methods】A total of 159 patients with kidney stones selected from June 2012 to June 2015 in our hospital was divided into a double channel group (N=77) and a single channel group (N=82). The patients of the double channel group were treated with the double channel PCNL and the patients of single channel group were treated with the single channel PCNL to observe the effects on renal hemodynamics of patients in the two groups before PCNL and after PCNL 30 days. 【Results】The operation time of the double channel group was significantly higher than that of the single channel group (P < 0.05); the bleeding amount and intraoperative rehydration amount of the double channel group were significantly lower than in the single channel group (P<0.05); After 30 days,Vmax and S / D of the renal artery,segmental artery,and interlobar artery was significantly higher than that of preopertative, and RI was significantly lower than that of preopertative (P<0.05). In the double channel group,Vmax and S / D of renal artery,segmental artery,interlobar artery was significantly higher than that of the single channel group; RI was significantly lower than that of the single channel group (P < 0.05). After the operation, GFR values were all significantly lower than before operation in both groups (P<0.05); the change of GFR values between preoperative and postoperative in the single channel group of patients was significantly greater than that of the double channel group (P<0.05). 【Conclusion】PCNL surgery has effects on renal hemodynamic to a degree. Because of less damage is caused on the blood vessels in the double channel PCNL process, the effects on the renal hemodynamic is smaller than that of the single channel PCNL.

Bioelectrical Impedance Analysis Evaluating Fluid Distribution in Patients with EndStage Renal Disease

【Objective】To investigate the bioelectrical impedance analysis evaluating fluid distribution in patients with endstage renal disease (ESRD). 【Methods】Thirty cases of patients with ESRD without hematodialysis (HD) from Jan. 2013 to Jun. 2015 were studied as the nonHD group and another 30 cases undergoing hematodialysis were studied as the HD group.Thirty cases undergoing health examination were used as the control group. Extracellular account body weight percentage (ECV%), intracellular fluid percentage of body weight (ICV), percentage of body weight (TBW%) accounted for the total body water, extracellular/intracellular fluid (ECV/ICV ), and edema index (ECV / TBW) of the cases were determined. 【Results】ECV%: decreased in all three groups; the decrease was most apparent in the nonHD group, less apparent in the HD group, and least apparent in the control group, with significant contrasts between the levels of any two groups (P<0.05). ICV%: there were no significant differences among the three groups (P>0.05). TBW %: those of NonHD group and HD group were not significantly different (P>0.05), although they were higher than that of the control group (P<0.05).. ECV/ICV: decreased most in the nonHD group, less in the HD group, and least in the control group, with significant differences between any two groups (P<0.05). ECV/TBW: decreased most in the nonHD group, less in the HD group, and least in the control group, with significant differences between any two groups (P<0.05). 【Conclusion】Bioelectrical impedance analysis in dialysis patients with endstage renal disease is effective. It will show that ESRD patients have significantly abnormal distribution of body fluids,.mainly, reflecting extracellular fluid increases but without significant effect on the intracellular fluid.Thus, bioelectrical impedance analysis will guide dialysis treatment for ESRD.

Prevalence of Diabetes among the Population in Changsha

【Objective】To investigate the prevalence and risk factors of diabetes mellitus (DM) in Changsha area.【Methods】A crosssectional survey was conducted among 2961 subjects aged 25 to 90 years in Changsha area and the physical examination collection was made on anthropometric data like height, body mass, waist circumference and hip circumference , plus the morning fasting oral glucose tolerance test (OGTT) and detection of blood glucose, blood lipids, uric acid and so on.【Results】With the increase of age, the prevalence of DM in Changsha increased significantly. Logstic stepwise regression analysis showed that male, age, high serum triglyceride and cholesterol, hypertension, family history of hypertension and central obesity in patients tend to be high risk of DM.【Conclusion】The prevalence rate of DM in Changsha area was getting higher, the family history of serum triglyceride , cholesterol , high blood pressure and hypertension were the risk factors of DM.

Study of Normal Reference Range of Children Upper Airway and Hyoid with No Obstructive Sleep Apnea Hypoventilation Syndrome (OSAHS) in Anyang

【Objective】The establishment of normal reference range of children upper airway and hyoid with no obstructive sleep apnea hypoventilation syndrome (OSAHS) in Anyang city.【Methods】400 children with no OSAHS clinical symptoms in the orthodontic Department of the Stomatology Hospital of Anyang city from January 2012 to December 2014 were selected as the research subjects (observation group) In addition, 150 cases with normal jaws(upper and lower) both sagittal and vertical performing an average angle relationship (osseous Type I) were selected as the control group. The two groups were all shot with cephalometric roentgenogram measurement(CRM).【Results】 The comparison between[subspinale(A) Nasal root point (N) supramental(B)angle, Angle ANB], the protrusion and mandibular plane angle (MPSN angle) of the two groups had no significant difference (P>0.05). PNSR、PNSAd2、PNSUPW、TBTPPW and VLPW of the two groups both for male and female had no significant difference (P>0.05). The soft palate backward,SPPSPPW and UMPW of male were significantly lower than those of female (P<0.05). Hyoidsella level distance (HS) of the two groups of the male and the female showed no significant difference (P>0.05). HFH,HMP and HVIof the male were significantly higher than those of the female(P<0.05).【Conclusion】This study establishes the normal reference range of the upper airway and hyoid bone of children's early permanent dentition in Anyang city; Hyoid position in the male is lower and more forward than that in the female ; The airway clearance after airway sagittal diameter of the soft palate of the male is smaller than that of male. Sagittal dimensions of upper airway soft palate in the posterior airway space of the male are smaller than those of the female.
