Value of CTP and Multiphase CTA in Evaluating Collateral Circulation in Patients with Ischemic Stroke

Abstract  【Objective】To investigate the value of CT perfusion imaging (CTP) combined with multiphase CT angiography (MP-CTA) in evaluating collateral circulation in patients with ischemic stroke. 【Methods】A total of 153 patients with ischemic stroke were examined by digital subtraction angiography (DSA). According to the DSA results,patients were divided into the good group (n=101,with good collateral circulation) and the poor group (n=52,with poor collateral circulation). All patients were examined with CTP and MP-CTA. The perfusion parameters of infarcted core area and ischemic penumbra (rMTT,rCBV,rTTP,rCBF) were compared between the two groups,and the consistency of collateral circulation in ischemic stroke patients was evaluated by CTP combined with MP-CTA and DSA. 【Results】The rMTT,rCBV and rTTP in the infarct core area in the poor group were lower than those in the good group (P<0.05). There was no significant difference in rCBF between the good group and the poor group (P>0.05). There was no significant difference in rMTT,rCBV and rTTP of ischemic penumbra between the two groups (P>0.05). The rCBF of the poor group was lower than that of the good group (P<0.05). CTP combined with MP-CTA had good consistency with DSA in evaluating collateral circulation in patients with ischemic stroke (Kappa=0.823,P=0.000). 【Conclusion】CTP combined with MP-CTA can evaluate the level of collateral circulation in patients with ischemic stroke to a certain extent,which is helpful to formulate a targeted treatment plan in time.
