摘要【目的】探讨阶段性营养平衡目标干预在接受机械通气辅助治疗的重症患者中的应用效果。【方法】选取2018年1月至2020年12月在本院接受机械通气辅助治疗的86例重症患者,按照1∶1分为对照组(采用临床常规营养支持干预)与观察组(采用阶段性营养平衡目标干预),每组43例。比较两组患者营养指标、血气分析指标、炎症因子水平以及临床预后指标。【结果】干预后,观察组患者营养指标、血气分析指标均显著优于对照组(P<0.05);炎症因子水平、肠内营养不耐受率、急性生理学和慢性健康状况评价Ⅱ(acute physiology and chronic health evaluationⅡ,APACHE Ⅱ)评分均显著低于对照组,肺部症状消失时间、机械通气时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。【结论】接受机械通气辅助治疗的重症患者采用阶段性营养平衡目标干预,可显著提高患者营养状况,降低炎症因子水平,改善患者预后。
Abstract:【Objective】To explore the effect of phased nutritional balance target intervention in critically ill patients receiving mechanical ventilation-assisted therapy.【Methods】A total of 86 patients with severe mechanical ventilation-assisted therapy admitted to our hospital from January 2018 to December 2020 were divided into the control group and the experimental group according to the ratio of 1∶1, with 43 cases in each group. The control group was given clinical routine nutritional support intervention, while the experimental group was given phased nutritional balance target intervention. The nutritional indexes, blood gas analysis results, inflammatory factors and clinical prognostic indicators of the two groups were compared. 【Results】After intervention, the nutritional index and blood gas analysis results of the experimental group were significantly higher than those of the control group(P<0.05). Inflammation factors, mid-intestinal nutritional intolerance rate, Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ) score, lung symptom disappear time, mechanical ventilation time, hospital stay and prognostic indicators in the experimental group were significantly lower than those in the control group. The differences were statistically significant (P<0.05). 【Conclusion】The use of phased nutritional balance target intervention for critically ill patients receiving mechanical ventilation-assisted therapy can significantly improve their nutritional status, reduce the level of inflammation and improve the prognosis of patients.
