幽门螺杆菌根除治疗前后患儿肠道菌群及免疫功能变化情况分析

摘要 【目的】探讨幽门螺杆菌(Hp)根除治疗前后患儿肠道菌群及免疫功能变化情况。【方法】回顾性分析2017年9月至2020年3月本院收治的91例Hp感染患儿的临床资料,所有患儿均采用Hp感染四联标准化根治方案。对比患儿根除治疗前后患儿肠道菌群、免疫功能变化及药物安全性情况。【结果】治疗后,大肠杆菌属、肠球菌属、梭菌属、副拟杆菌属水平相对丰度均高于治疗前(P<0.05),普氏菌属、双歧杆菌属、乳酸杆菌属、考拉杆菌属、拟杆菌属水平相对丰度均低于治疗前(P<0.05);患儿CD3+、CD4+、CD8+、CD4+/CD8+水平均高于治疗前(P<0.05),CD8+水平低于治疗前(P<0.05)。治疗期间患儿均未出现明显的不良反应,安全性良好。治疗后,患儿IgA、IgM、IgG水平均高于治疗前,差异有统计学意义(P<0.05)。【结论】Hp感染患儿根除治疗后肠道菌群结构发生改变,但可有效改善患儿免疫功能。

Abstract:【Objective】To explore the changes of intestinal microecology and immune function in children before and after Helicobacter pylori (Hp) eradication treatment. 【Methods】The clinical data of 91 children with HP infection admitted to the hospital from September 2017 to March 2020 were retrospectively analyzed. All children were treated with a quadruple standardized radical cure for HP infection. The changes of intestinal microecology and immune function in children before and after eradication treatment were compared. Drug safety was evaluated. 【Results】After treatment, the proportion of Escherichia coli, Enterococcus, Clostridium, and Parabacteroides was higher than before treatment (P<0.05), The proportion of Pretotella, Bifidobacterium, Lactobacillus, Koala, Bacteroides was lower than before treatment (P<0.05). CD3+, CD4+ and CD4+/CD8+ levels after treatment were higher than those before treatment (P<0.05), while CD8+ level after treatment was lower than before treatment (P<0.05). The levels of IgA, IgM and IgG after treatment in children were higher than before treatment (P<0.05). The children had no obvious adverse reactions, and the safety was good during the treatment. 【Conclusion】The structure of the intestinal flora changes in children with Hp infection after eradication treatment, which can effectively improve their immune function.
