术前风险评估及对症护理对老年复杂腹股沟疝患者手术安全性的影响及预后恢复情况分析

摘要【目的】探讨术前风险评估及对症护理对老年复杂腹股沟疝患者手术安全性的影响及预后恢复情况。【方法】选择2016年1月到2019年12月本院收治的88例老年复杂腹股沟疝患者,根据随机表法将其分为对照组(常规手术护理)和观察组(常规手术护理+术前风险评估及对症护理),每组各44例。比较两组患者胃肠功能恢复及生活质量改善情况,并记录两组患者并发症发生情况及复发率。【结果】对照组肛门排气时间、肠鸣音恢复时间、术后进食时间以及胃肠蠕动开始时间大于观察组,差异有统计学意义(P<0.05)。对照组并发症总发生率为27.27%(12/44),明显高于观察组的13.64%(6/44),差异有统计学意义(χ²=5.709,P=0.017＜0.05)。对照组复发率为15.91%(7/44),明显高于观察组4.55%(2/44),差异有统计学意义(χ²=7.026,P=0.008＜0.05)。对照组的生活质量量表SF-36评分总分为(70.11±3.25)分,显著低于观察组的(79.82±5.18),差异有统计学意义(t=10.533,P<0.05)。【结论】老年复杂腹股沟疝手术患者采用术前风险评估及对症护理的效果较好,并发症少,安全性高,值得临床推广应用。

Abstract：【Objective】To explore the effect of preoperative risk assessment and symptomatic nursing on the safety of operation and the recovery of prognosis in the elderly patients with complicated inguinal hernia.【Methods】From January 2016 to December 2019, 88 elderly patients with complex inguinal hernia were selected and divided into control group (routine operation nursing) and observation group (routine operation nursing + preoperative risk assessment and symptomatic Nursing) according to the random table method, 44 cases in each group. The recovery of gastrointestinal function and improvement of quality of life were compared between the two groups, and the incidence of complications and recurrence rate were recorded. 【Results】The anal exhaust time, bowel sounds recovery time, postoperative eating time and gastrointestinal peristalsis start time in the control group were longer than those in the observation group (P<0.05). The total incidence of complications in the control group was 27.27% (12/44), significantly higher than that in the observation group (13.64%) (6/44), the difference was statistically significant (χ²=5.709,P=0.017<0.05).The recurrence rate in the control group was 15.91% (7/44), which was significantly higher than 4.55%(2/44) in the observation group, and the difference was statistically significant (χ²=7.026,P=0.008<0.05). The total score SF-36 the quality of life scale in the control group was (70.11±3.25), which was significantly lower than that in the observation group (79.82±5.18), and the difference was statistically significant (t=10.533, P<0.05).【Conclusion】The results of preoperative risk assessment and symptomatic nursing for the elderly patients with complicated inguinal hernia operation are better, with fewer complications and high safety, which is worthy of clinical application.
