Abstract：【Objective】To investigate the clinical effect of laparoscopic left hepatectomy for intrahepatic bile duct stones.【Methods】The clinical data of 87 patients with intrahepatic bile duct stones admitted to our hospital from January 2013 to December 2017 were retrospectively analyzed. Among them, 42 cases were treated with laparoscopic left hemihepatectomy (the observation group) and 45 cases were treated with open abdominal left hemihepatectomy (the control group). The intraoperative and postoperative indicators, liver function index before and 3 days after operation , the residual rate of stones and complications of the two groups were compared. 【Results】The blood loss, intraoperative blood transfusion rate and incision length in the observation group were less than those in the control group (P<0.05). However, the operation time of the observation group was longer than that of the control group (P<0.05).Postoperative pain relief, hospitalization, anal exhaust time, bowel function recovery time and visual analogue scale (VAS) were better in the observation group than those in the control group (P<0.05).There was no significant difference in the levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) between the two groups before operation (P>0.05).The levels of AST and ALT in both groups increased on the 3rd day after operation, but the increase in the observation group was less than that in the control group (P<0.05).There were no significant differences between the two groups in terms of the residual rate of stone and the incidence of complications one week after operation (all P>0.05). 【Conclusion】Laparoscopic left hepatectomy for patients with the treatment of intrahepatic bile duct stones can not only ensure clinical efficacy, but also significantly reduce the amount of intraoperative bleeding, reduce pain in patients with less trauma and allow patients to recover faster.
