Abstract:【Objective】 To investigate the diagnostic value of high-sensitivity C-reactive protein (hs-CRP) combined with neutrophil-to-lymphocyte ratio (NLR) for urosepsis in patients after urinary calculus surgery. 【Methods】 A total of 80 patients who underwent urinary calculus surgery in our hospital from October 2020 to December 2024 were enrolled. According to the postoperative diagnosis of urosepsis, the patients were divided into sepsis group (n=26) and non-sepsis group (n=54). The baseline data, hs-CRP and NLR levels were compared between the two groups. Univariate and multivariate Logistic regression analyses were used to analyze the related factors of postoperative urosepsis. Receiver operating characteristic (ROC) curves were plotted to analyze the diagnostic efficacy of hs-CRP, NLR alone and their combination. 【Results】 There were statistically significant differences in gender, surgical approach, stone diameter, positive urine culture, operation time, NLR and hs-CRP levels between the two groups (P<0.05). Multivariate Logistic regression analysis showed that NLR, hs-CRP, surgical approach and positive urine culture were influencing factors of postoperative urosepsis (P<0.05). ROC curve analysis showed that the area under curve (AUC), sensitivity and specificity of hs-CRP were 0.937, 92.3% and 84.9% respectively; those of NLR were 0.876, 92.3% and 71.9% respectively; the combined detection achieved an AUC of 0.952, sensitivity of 92.3% and specificity of 90.4%. 【Conclusions】 Combined detection of hs-CRP and NLR has high diagnostic value for urosepsis after urinary calculus surgery, which can be used as effective indicators for early clinical identification of this complication.
