Epidemiological Analysis of Respiratory Infectious Diseases in a Military Academy from 2017 to 2023

【Objective】To analyze the epidemiological status of respiratory infectious diseases in a military academy from January 2017 to December 2023. 【Methods】Relevant data were extracted from the Public Health Emergency and Infectious Disease Epidemic Reporting Information System of the 921st Hospital of Joint Logistics Support Force, and processed and analyzed using Excel 2010 software. 【Results】From 2017 to 2023, a total of 2,874 cases of respiratory infectious diseases were reported in the military academy. Among them, the total number of cases from 2017 to 2019 was 236, mainly including influenza (47.46%, 112/236), pulmonary tuberculosis (25.00%, 59/236), and varicella (17.37%, 41/236); the total number of cases from 2020 to 2023 was 2638, mainly including influenza (62.93%, 1660/2638), COVID-19 infection (32.41%, 855/2638), and varicella (2.12%, 56/2638). Influenza accounted for 61.66% of the total respiratory infectious disease cases, and COVID-19 infection accounted for 29.75%. Most cases occurred in winter and spring, while the number of cases was relatively small from June to September. 【Conclusion】Although the number of respiratory infectious disease cases in the military academy fluctuates slightly, it shows an overall upward trend. Influenza, COVID-19 infection, varicella, and pulmonary tuberculosis are the most common types. Clinically, it is necessary to strengthen preventive education, epidemic monitoring, and disease control work.

The Effect of MUC3A on the Proliferation of Extrahepatic Cholangiocarcinoma Cells

【Objective】To explore the effect of MUC3A on the proliferation of extrahepatic cholangiocarcinoma (EHCC) cells. 【Methods】QBC939 cells with high MUC3A expression were selected for cell transfection. Transient transfection was used, and shRNA interference sequences (#1 and #2) targeting the MUC3A gene were designed and synthesized. Recombinant lentiviral plasmids were packaged to produce viral solution, which was then used to infect EHCC QBC939 cells. The cells were divided into four groups for culture: untransfected cells (controll), empty virus-transfected cells (shRNA-Ctrl), and lentivirus-mediated shRNA-transfected cells (shRNA-MUC3A#1, shRNA-MUC3A#2). Western Blot (WB) was used to detect the silencing efficiency of shRNA on the MUC3A gene. Colony formation assay was used to detect the effect of MUC3A knockdown on the proliferation ability of QBC939 cells. 【Results】WB results showed that compared with the control group, the MUC3A expression in the shRNA-MUC3A#1 and shRNA-MUC3A#2 groups was decreased, and shRNA-MUC3A#2 had a better silencing effect, with statistically significant differences (all P<0.05). Colony formation assay results showed that 24 hours after transfection, silencing MUC3A significantly reduced the colony formation of QBC939 cells; both shRNA-MUC3A#1 and shRNA-MUC3A#2 could inhibit the proliferation of QBC939 cells, and shRNA-MUC3A#2 had a stronger inhibitory effect, with statistically significant differences (all P<0.05), indicating that MUC3A knockdown affects the proliferation ability of QBC939 cells. 【Conclusion】 MUC3A plays an important role in the proliferation of EHCC cells; knocking down the expression of MUC3A protein in EHCC cells can inhibit the proliferation of extrahepatic cholangiocarcinoma cells.

Value of T Lymphocyte Subsets in Predicting Short-Term Outcomes in Elderly Patients with Severe Pneumonia

【Objective】To explore the predictive value of peripheral blood T lymphocyte subsets for short-term in-hospital outcomes in elderly patients with severe pneumonia. 【Methods】The clinical data of 80 elderly patients with severe pneumonia admitted to our hospital from June 2021 to November 2023 were retrospectively analyzed. All patients received conventional treatment and were divided into the poor outcome group (n=26) and the good outcome group (n=54) based on the therapeutic effect after 28 days. Feasibility Analysis of Transvaginal Posterior Fornix Extraction of Myomas after Laparoscopic Myomectomy

【Objective】To investigate the feasibility of extracting myomas via the transvaginal posterior fornix after laparoscopic myomectomy. 【Methods】 A total of 120 patients who underwent myomectomy in our hospital from January 2022 to December 2024 were selected as the study subjects. Based on the patients' voluntary choice of surgical method, they were divided into Group A, Group B, and Group C, with 40 patients in each group. In Group A, after laparoscopic myomectomy, the myomas were placed into the plastic packaging bag of a disposable suction device. Then, the posterior vaginal fornix was incised, the plastic bag was grasped, and the myomas were tractioned out. In Group B, after laparoscopic myomectomy, the puncture port was enlarged, and a morcellator was used to cut the myomas into small strips, which were then extracted through the puncture port. In Group C, patients underwent open abdominal myomectomy. The completion status of the surgery, operation time, intraoperative blood loss, scar size, incidence of postoperative complications, and Visual Analogue Scale (VAS) scores at 24 hours and 7 days postoperatively were compared among the groups.【Results】All surgeries were completed according to the preoperatively planned methods. The operation time in Group A was shorter than that in Groups B and C (P<0.05), but there was no statistically significant difference between Groups B and C (P>0.05). Intraoperative blood loss in Group A was less than that in Groups B and C (P<0.05), and blood loss in Group B was less than that in Group C (P<0.05). Scar size in Group A was smaller than that in Groups B and C (P<0.05), and scar size in Group B was smaller than that in Group C (P<0.05). VAS scores at 24 hours and 7 days postoperatively in Group A were lower than those in Groups B and C (P<0.05), and VAS scores in Group B were lower than those in Group C (P<0.05). There was no statistically significant difference in the incidence of postoperative complications among the three groups (P>0.05). 【Conclusion】 Extracting myomas via the transvaginal posterior fornix after laparoscopic myomectomy is a feasible surgical approach, characterized by shorter operation time, less blood loss, smaller scars, and milder pain. It aligns with the principles of minimally invasive surgery and oncological safety (no-tumor principle).

Effects of Different Urinary Diversion Surgeries on Urodynamic Parameters, Sexual Function and Quality of Life in Male Patients after Radical Cystectomy for Bladder Cancer

【Objective】To explore the effects of different urinary diversion surgeries on urodynamic parameters, sexual function and quality of life in male patients after radical cystectomy for bladder cancer. 【Methods】The clinical data of 90 male patients who underwent laparoscopic radical cystectomy in our hospital from January 2024 to December 2024 were analyzed retrospectively. According to the type of urinary diversion surgery, the patients were divided into the observation group (undergoing orthotopic ileal neobladder surgery) and the control group (undergoing ileal conduit surgery), with 45 cases in each group. The clinical indicators, urodynamic parameters, sexual function [International Index of Erectile Function-5 (IIEF-5) score, Arizona Sexual Experience Scale (ASEX) score] and quality of life [36-Item Short Form Health Survey (SF-36) score] were compared between the two groups. 【Results】There were no statistically significant differences in intraoperative blood loss, hospital stay and intestinal function recovery time between the two groups (P>0.05), while the operation time of the observation group was longer than that of the control group, with a statistically significant difference (P<0.05). There were no statistically significant differences in postoperative neobladder capacity and postoperative residual urine volume between the two groups (P>0.05). The postoperative maximum urinary flow rate, daytime urinary control rate and nighttime urinary control rate in the observation group were all higher than those in the control group (P<0.05). After surgery, the IIEF-5 score of the observation group was higher than that of the control group, and the ASEX score was lower than that of the control group (P<0.05). There were no statistically significant differences in scores of bodily pain, social function and role-emotional between the two groups (P>0.05). The scores of physical function, role-physical, general health, mental health and total score in the observation group were all higher than those in the control group (P<0.05). 【Conclusion】Orthotopic ileal neobladder surgery can improve the urodynamic parameters of male patients undergoing laparoscopic radical cystectomy for bladder cancer, and has less impact on patients' postoperative sexual function and quality of life.

Analysis of the Efficacy and Safety of Haematococcus Pluvialis and Snow Frog Egg Peptide Solid Beverage in the Treatment of Patients with Diminished Ovarian Reserve and Menstrual Disorders

【Objective】To explore the efficacy and safety of Haematococcus pluvialis and snow frog egg peptide solid beverage in the treatment of menstrual disorders in patients with diminished ovarian reserve (DOR). 【Methods】Patients with DOR and menstrual disorders who visited our hospital from April 2022 to February 2024 were selected. Among them, patients treated with Haematococcus pluvialis and snow frog egg peptide solid beverage were assigned to the observation group (n=30), and those treated with coenzyme Q10 capsules were assigned to the control group (n=30). The menstrual cycle, menstrual volume, quality of life, medication compliance, satisfaction, and adverse reactions before and after treatment were compared between the two groups. 【Results】After treatment, the menstrual cycle and menstrual volume in the observation group were better than those in the control group (P<0.05). There were no statistically significant differences in quality of life scores, medication compliance scores, and patient satisfaction between the two groups (P>0.05). During the treatment period, no adverse reactions caused by the drugs in this study were found in either group. 【Conclusion】Haematococcus pluvialis and snow frog egg peptide solid beverage can improve the menstrual cycle and menstrual volume in patients with DOR and menstrual disorders, maintain high medication compliance and satisfaction during use, and have high safety.

Therapeutic Effect of Banxia Xiexin Decoction on Atrophic Gastritis via Regulation of the TLR4/NF-κB Signaling Pathway

【Objective】To investigate the therapeutic effect of Banxia Xiexin Decoction on atrophic gastritis (CAG) by regulating the Toll-like receptor 4 (TLR4)/nuclear factor-kappa B (NF-κB) signaling pathway.【Methods】 A total of 104 patients with CAG were randomly divided into two groups, with 52 cases in each. The control group received conventional Western medicine treatment, while the observation group received Banxia Xiexin Decoction in addition to the treatment used in the control group. The two groups were compared in terms of clinical efficacy, traditional Chinese medicine (TCM) symptom scores, immune function indicators, expression levels of the TLR4/NF-κB signaling pathway, inflammatory factors [interleukin-2 (IL-2), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α)], and incidence of adverse reactions.【Results】The total effective rate in the observation group was significantly higher than that in the control group (P<0.05). After treatment, TCM symptom scores in both groups were significantly reduced compared to before treatment (P<0.05), with lower TCM scores observed in the observation group than in the control group (P<0.05). levels decreased in both groups after treatment (P<0.05), with more pronounced improvement in immune function indicators in the observation group (P<0.05). Expression levels of TLR4 mRNA, NF-κB mRNA, and IL-2, IL-6, TNF-α were lower in both groups post-treatment compared to pre-treatment levels, with the observation group showing greater reductions than the control group (P<0.05). The difference in the overall incidence of adverse reactions between the two groups was not statistically significant (P>0.05).【Conclusion】 Banxia Xiexin Decoction in the treatment of CAG patients can significantly improve the clinical effective rate, TCM symptom scores, and immune function indicators. It also effectively alleviates inflammatory responses by regulating the expression of the TLR4/NF-κB signaling pathway, with a high level of safety.

Clinical Application Effect of Traditional Chinese Medicine Poultice Combined with Abdominal Massage in Patients with Incomplete Intestinal Obstruction

【Objective】To explore the clinical application effect of Traditional Chinese Medicine (TCM) poultice combined with abdominal massage in patients with incomplete intestinal obstruction. 【Methods】 A total of 80 patients with incomplete intestinal obstruction admitted to our hospital from September 2022 to December 2024 were selected and divided into the control group and the observation group according to the random number table method, with 40 patients in each group. The average treatment days, recovery time of bowel sounds, time to first flatus, time to first defecation, time to first oral intake, conversion to surgery rate, pain degree [Visual Analogue Scale (VAS) score] and quality of life [Gastrointestinal Quality of Life Index (GIQLI) score] were compared between the two groups. 【Results】The average treatment time, recovery time of bowel sounds, time to first flatus, time to first defecation and time to first oral intake in the observation group were shorter than those in the control group, and the conversion to surgery rate was lower than that in the control group, with statistically significant differences (P<0.05). At 3 days and 5 days after intervention, the VAS scores of both groups were lower than those before intervention, with statistically significant differences (P<0.05); at 5 days after intervention, the VAS score of the observation group was lower than that of the control group, with a statistically significant difference (P<0.05). One month after intervention, the GIQLI scores of both groups were higher than those before intervention, and the observation group was higher than the control group, with statistically significant differences (P<0.05). 【Conclusion】 TCM poultice combined with abdominal massage can significantly improve the clinical symptoms of patients with incomplete intestinal obstruction, shorten the treatment time, reduce the conversion to surgery rate, and improve the quality of life, which has good clinical application value.

Clinical Diagnostic Value of Routine Blood Test-Related Indicators in Hepatitis, Cirrhosis, and Hepatocellular Carcinoma Associated with HCV Infection

【Objective】 To investigate the auxiliary diagnostic value of routine blood test-related indicators in the progression of chronic liver diseases (hepatitis, cirrhosis, hepatocellular carcinoma) associated with hepatitis C virus (HCV) infection.【Methods】 A total of 215 patients with HCV infection were enrolled and divided into the hepatitis group (60 cases), the cirrhosis group (95 cases), and the hepatocellular carcinoma group (60 cases) according to disease stage. Differences in routine blood test-related indicators were compared among the three groups. Receiver operating characteristic (ROC) curves were used to evaluate the diagnostic efficacy of individual and combined indicators for disease staging.【Results】 Significant differences (all P<0.05) were observed among the three groups in platelet count (PLT), plateletcrit (PCT), lymphocyte count (L), neutrophil count (N), neutrophil-to-lymphocyte ratio (NLR), and platelet-to-neutrophil ratio (PNR). The combined detection of L + PLT + PNR improved diagnostic performance in distinguishing hepatitis from cirrhosis, while N + PCT enhanced sensitivity in differentiating cirrhosis from hepatocellular carcinoma.【Conclusion】 Routine blood test-related indicators provide important reference value for staging diagnosis of HCV-associated liver diseases. Combined testing can optimize differential diagnosis of hepatitis, cirrhosis, and hepatocellular carcinoma in HCV-infected patients, offering a simple and cost-effective laboratory basis for clinical practice.

Clinical Efficacy of Scalp-Ear Moxibustion Combined with Auricular Point Sticking in the Treatment of Patients with Meniere's Disease of Qixu Xueyu Type

【Objective】To explore the clinical efficacy of scalp-ear moxibustion combined with auricular point sticking in the treatment of patients with Meniere's disease of qixu xueyu type. 【Methods】A total of 60 patients with Meniere's disease of qixu xueyu type admitted to our hospital from March 2023 to October 2024 were selected and divided into an observation group and a control group according to the random number table method, with 30 cases in each group. The control group was treated with betahistine hydrochloride, while the observation group was given scalp-ear moxibustion combined with auricular point sticking on the basis of the control group. Both groups were treated for 4 weeks. The TCM syndrome scores, as well as scores of vertigo, tinnitus and quality of life, were compared between the two groups before and after treatment.【Results】After treatment, the scores of primary and secondary TCM syndromes, Dizziness Handicap Inventory (DHI) score and Tinnitus Handicap Inventory (THI) score in the observation group were lower than those in the control group, while the Meniere's Disease Outcome Questionnaire (MDOQ) score was higher than that in the control group, with statistically significant differences (P<0.05). 【Conclusion】Scalp-ear moxibustion combined with auricular point sticking in the treatment of patients with Meniere's disease of qixu xueyu type can significantly improve patients' vertigo and tinnitus symptoms, enhance their quality of life, and exert a good effect on improving TCM symptoms. It suggests that this treatment method can be used as an effective adjuvant therapy for patients with Meniere's disease.

Effect of Routine Balanced Total Nutrient Preparation Combined with Whey Protein-Enhanced Nutritional Support on Nutritional Status and Immune Function in Patients with Severe Traumatic Brain Injury

【Objective】To explore the effect of routine balanced total nutrient preparation combined with whey protein-enhanced nutritional support on the nutritional status and immune function of patients with severe traumatic brain injury (STBI). 【Methods】The clinical data of 90 STBI patients receiving nasogastric feeding in our hospital from May 2021 to June 2024 were retrospectively analyzed. They were divided into the enhanced group (n=46) and the routine group (n=44) according to different nutritional support methods. The routine group was given nutritional support with routine balanced total nutrient preparation, while the enhanced group was given nutritional support with routine balanced total nutrient preparation combined with whey protein enhancement. Before and after intervention, the nutritional status [Mini Nutritional Assessment (MNA) score], serum nutritional indicators [total protein (TP), prealbumin (PA), albumin (Alb)], immune indicators [immunoglobulin A (IgA), immunoglobulin M (IgM), immunoglobulin G (IgG)], prognosis and incidence of adverse reactions were compared between the two groups.【Results】After intervention, the MNA scores of both groups were higher than those before intervention, and the enhanced group was higher than the routine group, with a statistically significant difference (P<0.05). After intervention, the levels of TP, PA and Alb in both groups were higher than those before intervention, and the enhanced group was higher than the routine group, with statistically significant differences (P<0.05). After intervention, the levels of IgA, IgM and IgG in the enhanced group were higher than those in the routine group, with statistically significant differences (P<0.05). The length of hospital stay in the enhanced group was shorter than that in the routine group, with a statistically significant difference (P<0.05); after intervention, the National Institutes of Health Stroke Scale (NIHSS) score of the enhanced group was lower than that of the routine group, and the Activity of Daily Living (ADL) score was higher than that of the routine group, with statistically significant differences (P<0.05). The incidence of adverse reactions in the enhanced group and the routine group was 32.61% (15/46) and 38.64% (17/44) respectively, with no statistically significant difference (P>0.05).【Conclusion】Routine balanced total nutrient preparation combined with whey protein-enhanced nutritional support can improve the nutritional status of STBI patients, enhance immune function, promote the recovery of neurological function, improve the ability of daily living, and has good safety.

Clinical Effect of Epley's Canalith Repositioning Maneuver Combined with Traditional Chinese Medicine Filiform Needle Acupuncture in the Treatment of Patients with Benign Paroxysmal Positional Vertigo

【Objective】To explore the clinical effect of Epley canalith repositioning maneuver combined with filiform needle acupuncture in the treatment of patients with benign paroxysmal positional vertigo (BPPV). 【Methods】A total of 93 patients with BPPV admitted to our hospital from February 2021 to February 2023 were selected as the research subjects and randomly divided into the control group (n=46, treated with Epley canalith repositioning maneuver) and the observation group (n=47, treated with Epley canalith repositioning maneuver combined with filiform needle acupuncture). The blood rheological parameters (high shear whole blood viscosity, low shear whole blood viscosity, plasma viscosity), negative emotions [Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) scores], balance function [Berg Balance Scale (BBS) score], and quality of life [Dizziness Handicap Inventory (DHI) score] of the two groups were compared before treatment, 1 week after treatment, and 2 weeks after treatment. 【Results】After 2 weeks of treatment, the high shear whole blood viscosity, low shear whole blood viscosity, plasma viscosity, HAMA score, and HAMD score of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05). The BBS score of the observation group was higher than that of the control group 1 week and 2 weeks after treatment, and the differences were statistically significant (P<0.05). After 2 weeks of treatment, the scores of all dimensions of DHI in both groups were lower than those before treatment and 1 week after treatment. and the scores of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05). 【Conclusion】Epley canalith repositioning maneuver combined with filiform needle acupuncture can effectively improve the blood rheological parameters, negative emotions, and balance function of BPPV patients, thereby improving their quality of life.

Analysis of Risk Perception and its Influencing Factors in Pregnant Women at High Risk for Gestational Diabetes Mellitus

【Objective】To investigate the current status of risk perception and its influencing factors among pregnant women at high risk for gestational diabetes mellitus (GDM). 【Methods】A total of 286 pregnant women identified as high risk for GDM and undergoing regular prenatal checkups at Yancheng Maternal and Child Health Hospital between December 2024 and February 2025 were enrolled. Data were collected using a general information questionnaire, the Diabetes Management Self-Efficacy Scale, the Gestational Diabetes Mellitus Self-Management Scale, the Knowledge-Attitude-Practice Questionnaire for high-risk GDM women, the Risk Perception Scale for pregnant women with GDM, and the Social Support Scale for GDM patients. Multivariate analysis was performed to explore factors affecting risk perception. 【Results】Parity, pre-pregnancy body mass index (BMI), educational level, household income, diabetes self-efficacy, diabetes self-management ability scale score, knowledge-attitude-practice questionnaire score, and social support scale score were identified as independent predictors of risk perception in high-risk GDM pregnant women (P<0.05). 【Conclusion】Parity, pre-pregnancy BMI, education level, family income, self-efficacy, self-management ability, knowledge-attitude-practice level, and social support are significant independent factors influencing risk perception in high-risk pregnant women for GDM. Enhanced health education and social support interventions are recommended to improve risk perception and promote early prevention and management of GDM.

Analysis of the Relationship between Serum Reproductive-Related Biomarker Levels and Fertility Index in Patients with Endometriosis

【Objective】To investigate the correlations between the levels of C1q/TNF-related protein 3 (CTRP3), Apelin, and inhibin B (INHB) in the serum of patients with endometriosis and their reproductive indices. 【Methods】From January 2022 to October 2024, 107 patients with endometriosis who visited our hospital were selected as the observation group, and 52 healthy women who underwent physical examinations in the same period at our hospital were selected as the control group. The levels of CTRP3, Apelin, and INHB in the serum of both groups were detected. The reproductive ability was evaluated using the reproductive index score. Spearman correlation analysis was used to analyze the correlations between the levels of CTRP3, Apelin, and INHB and the reproductive index. 【Results】The levels of CTRP3 and INHB in the observation group were significantly lower than those in the control group, while the level of Apelin was significantly higher than that in the control group. The differences were statistically significant (P<0.05). The reproductive index of the observation group was significantly lower than that of the control group, and the difference was statistically significant (P<0.05). Correlation analysis showed that the level of Apelin was negatively correlated with the reproductive index score (

Application Effect of One-on-One Doula Accompaniment Combined with Perineal Massage in Primiparas' Childbirth

【Objective】To explore the application effect of one-on-one doula accompaniment combined with perineal massage in primiparas' childbirth. 【Methods】A total of 106 primiparas who delivered vaginally in our hospital from November 2023 to November 2024 were selected and divided into an observation group and a control group according to the random number table method, with 53 cases in each group. The control group received routine nursing, while the observation group received one-on-one doula accompaniment combined with perineal massage nursing. The anxiety emotion [Hamilton Anxiety Scale (HAMA) score], childbirth well-being [General Well-Being Schedule (GWB) score], pain degree [Visual Analogue Scale (VAS) score], neonatal health status (neonatal Apgar score), labor duration, degree of perineal laceration and postpartum 2-hour blood loss were compared between the two groups. 【Results】The HAMA score and VAS score of the observation group were lower than those of the control group, the GWB score was higher than that of the control group, and the labor duration was shorter than that of the control group, with statistically significant differences (P<0.05). The perineal integrity rate of the observation group was higher than that of the control group, and the degree of perineal laceration was milder than that of the control group, with a statistically significant difference (P<0.05); the postpartum 2-hour blood loss of the observation group was less than that of the control group, with a statistically significant difference (P<0.05). There was no statistically significant difference in the neonatal Apgar score between the two groups (P>0.05). 【Conclusion】One-on-one doula accompaniment combined with perineal massage can reduce the anxiety and pain of primiparas, improve childbirth well-being, shorten labor duration, reduce the degree of perineal injury and postpartum 2-hour blood loss, and has high safety.

Clinical Value of Simplified Contrast-Enhanced CT in the Differential Diagnosis of Adrenal Adenoma and Adrenal Pheochromocytoma

【Objective】To explore the clinical value of simplified contrast-enhanced CT in the differential diagnosis of adrenal adenoma and adrenal pheochromocytoma. 【Methods】The clinical data of 60 patients with pathologically confirmed adrenal tumors admitted to our hospital from August 2021 to December 2024 were analyzed retrospectively. The patients were divided into the adrenal adenoma group and the adrenal pheochromocytoma group according to different tumor types, with 30 cases in each group. The CT imaging features and CT density parameters of the two groups were compared, and the receiver operating characteristic (ROC) curve was used to analyze the value of simplified contrast-enhanced CT in the differential diagnosis of the two types of tumors. 【Results】 There were statistically significant differences in the shape and cystic degeneration between the two groups (P<0.05); there was no statistically significant difference in calcification (P>0.05). The plain CT value, portal venous phase CT value, absolute enhancement value, relative enhancement ratio, long diameter of the lesion, and short diameter of the lesion in the adrenal pheochromocytoma group were all higher than those in the adrenal adenoma group, with statistically significant differences (P<0.05). ROC curve analysis showed that the long diameter of the lesion had the best diagnostic efficiency [area under the curve (AUC)=0.913], with a sensitivity of 86.7% and a specificity of 83.3% when the cut-off value was 33.805 mm; the sensitivity was the highest (93.3%) when the portal venous phase CT value was 102.15 HU, and the specificity was the highest(93.3%) when the plain CT value was 28.38 HU. 【Conclusion】Simplified contrast-enhanced CT has important clinical application value in the differential diagnosis of adrenal adenoma and adrenal pheochromocytoma, and the combination of the long diameter of the lesion and the portal venous phase CT value can be used as a reliable indicator for differentiating adrenal adenoma and pheochromocytoma.

Expression and Clinicopathological Significance of Mismatch Repair Proteins and p53 in Patients with Colorectal Cancer

【Objective】To investigate the expression of mismatch repair proteins (MMRP) and p53 in patients with colorectal cancer and their clinicopathological significance.【Methods】 Ninety-eight patients with colorectal cancer were included. According to the presence or absence of MMRP expression, they were divided into the dMMR group (n=20) and the pMMR group (n=78). According to p53 expression, they were divided into the p53-positive group (n=72) and the p53-negative group (n=26). General clinical data were compared between groups, and Spearman correlation analysis was performed to evaluate the relationships among MLH1, PMS2, MSH2, and MSH6 within the MMRP family.【Results】 Tumor diameter was larger in the pMMR group than in the dMMR group. The proportions of high differentiation, TNM stage Ⅲ~Ⅳ, intravascular tumor thrombus, and lymph node metastasis were higher in the pMMR group compared with the dMMR group, with statistically significant differences (P<0.05). In the p53-positive group, the proportions of male patients and TNM stage Ⅰ~Ⅱ were higher than those in the p53-negative group (P<0.05). Among the 20 patients in the dMMR group, MLH1 loss was observed in 6 cases, PMS2 in 7 cases, MSH2 in 3 cases, and MSH6 in 2 cases; MLH1 combined with PMS2 loss was found in 1 case, and MSH2 combined with MSH6 loss was shown in 1 case. MLH1 was significantly positively correlated with PMS2, and MSH2 with MSH6 (both P<0.05).【Conclusion】 Loss of MMRP expression is significantly associated with clinicopathological features such as tumor diameter and degree of differentiation in colorectal cancer patients. p53 positivity is significantly associated with patient gender and TNM stage.

【Objective】To explore the application effect of birth ball free position intervention combined with LK Pregnancy and Childbirth Basic Massage Program in midwifery for term primiparas. 【Methods】A total of 60 term primiparas who delivered in our hospital from February to August 2023 were selected and divided into the control group and the observation group according to the random number table method, with 30 cases in each group. The control group received birth ball free position intervention, while the observation group received birth ball free position intervention combined with LK Pregnancy and Childbirth Basic Massage Program. The labor duration, labor pain [Pain Rating Index (PRI), Visual Analogue Scale (VAS), Present Pain Intensity (PPI)], incidence of adverse events, and maternal satisfaction with nursing care were compared between the two groups. 【Results】After intervention, the duration of the first stage, second stage, third stage, and total labor of the observation group was shorter than that of the control group; the PRI, VAS, and PPI scores were lower than those of the control group; the total incidence of adverse reactions was lower than that of the control group; all differences were statistically significant (P<0.05). The maternal satisfaction with nursing care in the observation group was higher than that in the control group (P<0.05).【Conclusion】Birth ball free position intervention combined with LK Pregnancy and Childbirth Basic Massage Program can effectively shorten labor duration, reduce labor pain, lower the incidence of adverse events, and improve maternal satisfaction with nursing care.

Significance of Capillary Centrifugation Method in Identification of RhCcEe Blood Group in Patients with Chronic Aplastic Anemia after Transfusion

【Objective】To investigate the significance of the capillary centrifugation method in identification of RhCcEe blood group in patients with chronic aplastic anemia (CAA) after blood transfusion.【Methods】 A total of 103 post-transfusion CAA patients were tested for RhCcEe blood group using three methods: microcolumn gel card method, capillary centrifugation method, and improved capillary centrifugation method. The results were compared with those of the polymerase chain reaction-sequence specific primer (PCR-SSP) method.【Results】 In RhCcEe blood group detection, the detection rate of the improved capillary centrifugation method was 71.84% (74/103), which was higher than that of the capillary centrifugation method (46.60%, 48/103) and the microcolumn gel card method (18.45%, 19/103), with statistically significant differences (P<0.05). The detection rate of the capillary centrifugation method was higher than that of the microcolumn gel card method (P<0.05). Among CAA patients, the detection rate of RhCcEe blood group by the improved capillary centrifugation method was 25.00% in those with transfusion interval <14 days, which was lower than those with transfusion interval of 14-28 days (75.29%) and interval >28 days (80.00%) (P<0.05). No significant difference was found between patients with intervals of 14-28 days and those with >28 days (P>0.05).【Conclusion】In RhCcEe blood group identification of CAA patients, the improved capillary centrifugation method shows a relatively higher detection rate. Moreover, the detection rate increases with the extension of the transfusion interval, making it suitable for rapid auxiliary diagnosis of RhCcEe blood group in clinical practice.

Clinical Value of Plasma NT-proBNP Combined with Electrocardiogram in Predicting Pulmonary Hypertension Associated with Congenital Heart Disease

【Objective】To study the clinical value of plasma N-terminal pro brain natriuretic peptide (NT-proBNP) combined with electrocardiogram (ECG) in predicting pulmonary hypertension (PAH) associated with congenital heart disease (CHD). 【Methods】Sixty-two patients with simple CHD in our hospital from February 2018 to February 2021 were selected as the CHD group, and 53 patients with both CHD and PAH in the same period were selected as the CHD-PAH group. The general data of all patients were counted, the plasma NT-proBNP level was detected, and the ECG was detected. The general data, plasma NT-proBNP level and ECG parameters were compared between the two groups. The influencing factors of CHD complicated with PAH were analyzed by logistic regression; Receiver operating characteristic (ROC) was used to evaluate the predictive value of plasma NT-proBNP combined with ECG for CHD complicated with PAH. 【Results】There were no significant differences in sex, age, BMI, HR, SBP, DBP and defect type between the two groups (P>0.05). The proportion of pulmonary systolic pressure and NYHA class Ⅲ ~ Ⅳ in the CHD-PAH group was higher than that in the CHD group, and the 6-minute walking distance (6MWD) in the CHD-PAH group was shorter than that in the CHD group (P<0.05). The plasma NT-proBNP level, QTc interval and RV5 amplitude in the CHD-PAH group were higher than those in the CHD group (P<0.05). Logistic regression analysis showed that pulmonary artery systolic pressure, NYHA cardiac function grade, 6min walking distance, plasma NT-proBNP level, QTc interval and RV5 amplitude were the influencing factors of CHD complicated with PAH (P<0.05). The ROC analysis showed that the cut off values of NT-proBNP, QTc interval and RV5 amplitude predicted CHD combined with PAH were 712.68 ng/L, 418.59 ms and 2.09 mv, respectively, and their AUC values were 0.883, 0.875 and 0.848, respectively. And the AUC value of joint detection (NT-proBNP combined with ECG) was 0.957. 【Conclusion】Plasma NT-proBNP combined with ECG has certain predictive value for CHD complicated with PAH, which has certain clinical significance.

Reliability and Validity Analysis of the Chinese Version of HINE Scale in the Screening of High-Risk Infants for Cerebral Palsy

【Objective】To explore the reliability and validity of the Chinese version of the Hammersmith Infant Neurological Examination (HINE) Scale in the screening of high-risk infants for cerebral palsy. 【Methods】A total of 55 high-risk infants with cerebral palsy (the observation group) admitted to the pediatric department of Quanzhou First Hospital Affiliated to Fujian Medical University from June 2018 to June 2020 were selected, and 35 normal infants who underwent physical examination in our hospital during the same period were selected as the control group. The Chinese version of HINE was used to conduct a questionnaire survey, and the principal component analysis method was applied to test the reliability and validity of the scale. The scores of the Chinese version of HINE between the two groups were compared. 【Results】There were no statistically significant differences in birth weight, gestational age, gender, HINE screening age, and the proportion of premature and full-term infants between the observation group and the control group (P>0.05). The scores of neurological function, movement, posture, muscle tone, reflex response and total score in the observation group were all lower than those in the control group, with statistically significant differences (P<0.05). The content validity index of the Chinese version of HINE was all greater than 0.957, the content reliability coefficient of the total score of the Chinese version of HINE was 0.792, and the content reliability coefficients of the 5 items ranged from 0.786 to 0.898. The test-retest reliability coefficient related to the total score of the scale was 0.962, and the test-retest reliability coefficients related to the scores of the 5 items ranged from 0.945 to 0.994 (P<0.05). 【Conclusion】The Chinese version of HINE has high reliability and validity, which can be used for the early screening of high-risk infants with cerebral palsy and is worthy of clinical promotion.

Effect of Auricular Point Pressure Combined with Acupoint Application on Blood Pressure Fluctuation and Vascular Endothelial Function in Elderly Patients with Hypertension

【Objective】 To investigate the effect of auricular point pressure combined with acupoint application on blood pressure fluctuation and vascular endothelial function in elderly patients with hypertension. 【Methods】 A total of 106 elderly patients with essential hypertension were randomly divided into the control group and the observation group, with 53 cases in each. The control group received conventional antihypertensive medication and health education, while the observation group received additional auricular point pressure and acupoint application therapy for 12 weeks. The therapeutic efficacy, traditional Chinese medicine (TCM) syndrome scores, 24-hour ambulatory blood pressure parameters, and vascular endothelial function indices were compared between the two groups before and after intervention. 【Results】 The total effective rate in the observation group was 94.34%, significantly higher than 81.13% in the control group (P<0.05). After intervention, TCM syndrome scores, blood pressure level, plasma endothelin-1 (ET-1) and thromboxane A2 (TXA2) levels in the observation group were lower than those in the control group, while nitric oxide (NO) level in the observation group was higher than that in the control group (all P<0.05). 【Conclusion】 Auricular point pressure combined with acupoint application can effectively stabilize blood pressure fluctuations and improve vascular endothelial function in elderly hypertensive patients. This combined non-pharmacological TCM therapy shows good adjunctive clinical value and is worthy of wider clinical promotion.

【Objective】To explore the clinical significance of cardiac color Doppler ultrasound parameters combined with serum meteorin-like protein (Metrnl) level in evaluating cardiac function and condition of patients with heart failure. 【Methods】A total of 120 patients with heart failure (the observation group) admitted to our hospital from October 2021 to October 2024 were selected. According to the New York Heart Association (NYHA) cardiac function classification, the observation group was divided into grade Ⅱ group (n=62) and grade Ⅲ+Ⅳ group (n=58). In addition, 50 healthy volunteers who underwent physical examination in our hospital during the same period were selected as the control group. The cardiac color Doppler ultrasound parameters and serum Metrnl levels of different groups were compared, and the receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of cardiac color Doppler ultrasound parameters and serum Metrnl level for cardiac function and condition of patients with heart failure. 【Results】The left ventricular ejection fraction (LVEF) and serum Metrnl level in the observation group were lower than those in the control group, while the left atrial diameter (LAD) and left ventricular end-diastolic diameter (LVEDD) were higher than those in the control group, with statistically significant differences (P<0.05). The LVEF and serum Metrnl level in the grade Ⅱ group were higher than those in the grade Ⅲ+Ⅳ group, while the LAD and LVEDD were lower than those in the grade Ⅲ+Ⅳ group, with statistically significant differences (P<0.05). ROC curve results showed that the area under the curve (AUC) of the combination of LVEF, LAD, LVEDD and Metrnl in predicting cardiac function of patients with heart failure was significantly better than that of each index alone (P<0.05). 【Conclusion】Cardiac color Doppler ultrasound parameters combined with serum Metrnl level have good clinical application value in evaluating cardiac function and condition of patients with heart failure.

【Objective】 To investigate the efficacy of rituximab combined with belimumab in the treatment of refractory systemic lupus erythematosus (SLE) complicated with thrombotic microangiopathy (TMA). 【Methods】 A total of 80 patients with refractory SLE and TMA were randomly divided into the rituximab monotherapy group (the control group, 37 cases) and the rituximab combined with belimumab group (the combination group, 43 cases). Both groups received treatment for 12 weeks. The therapeutic efficacy, changes in SLE Disease Activity Index (SLEDAI), hematological parameters [white blood cell count (WBC), platelet count (PLT)], renal function indicators [serum creatinine (SCr), urinary red blood cells], immune function markers [complement components 3(C3) and complement components 4(C4)], inflammatory markers [erythrocyte sedimentation rate (ESR), high-sensitivity C-reactive protein (hs-CRP)], and adverse reactions were compared between the two groups before and after treatment. 【Results】 After treatment, the total effective rate in the combination group was 93.02%, significantly higher than 72.97% in the control group (P<0.05). Compared with baseline, both groups showed improvement in all clinical parameters after treatment (P<0.05), with the combination group showing more significant improvements in SLEDAI, WBC, PLT, albumin levels, C3, ESR, and hs-CRP compared with the control group (P<0.05). No significant differences were observed between the two groups in urinary protein, SCr, and urinary red blood cell counts (P>0.05). The incidence of adverse reactions did not differ significantly between groups (P>0.05). 【Conclusion】 Rituximab combined with belimumab in patients with refractory SLE complicated with TMA significantly improves clinical efficacy, hematological and renal function parameters, while maintaining good safety.

Accuracy and Clinical Significance of MALDI-TOF MS in Identifying Acinetobacter Hospitalis and Acinetobacter Baumannii

【Objective】To investigate the accuracy of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) in identifying Acinetobacter hospitalis (A. hospitalis) and Acinetobacter baumannii (A. baumannii), and to analyze the clinical features and antibiotic resistance differences between the two species in order to evaluate the impact of misidentification on clinical diagnosis and treatment.【Methods】MALDI-TOF MS results were compared with those of first-generation sequencing in 18 patients infected with A. hospitalis and 115 patients infected with A. baumannii. Their clinical characteristics and antimicrobial susceptibility data were analyzed.【Results】MALDI-TOF MS achieved an identification accuracy of 100.0% for A. baumannii, but only 66.67% for A. hospitalis, with an overall accuracy rate of 93.98%. Patients with A. hospitalis infection had significantly longer hospital stays than those with A. baumannii infection (P<0.05). Resistance rates to most antibiotics were higher in A. baumannii than in A. hospitalis (P<0.05).【Conclusion】MALDI-TOF MS demonstrates high accuracy in identifying A. baumannii but a relatively high misidentification rate for A. hospitalis, which is often incorrectly identified as A. baumannii. Although the clinical infection characteristics of the two species are similar, A. hospitalis infections are associated with longer hospitalization and significantly lower antibiotic resistance compared to A. baumannii. Therefore, when the two cannot be reliably distinguished in clinical practice, antimicrobial susceptibility testing should be performed whenever possible to guide antibiotic therapy.
