Correlation between Weight Change and Delayed Lactation in Pregnant Women with Diabetes

Abstract  【Objective】To explore the correlation between the weight change of pregnant women with diabetes and the delayed start of lactation.【Methods】The clinical data of 90 pregnant women with diabetes who were admitted to the Department of Obstetrics and Gynecology of the Xianyang Central Hospital in Shaanxi Province from January 2019 to January 2022 were retrospectively analyzed. According to whether there was a delay in the initiation of lactation,they were divided into delayed group (n=21) and non delayed group (n=69). We collected clinical data on the age of all pregnant women,whether insulin therapy was used,whether they had first delivery,pre pregnancy body mass index (BMI),weight gain during pregnancy,whether they had a cesarean section,whether they were premature,whether they were anxious or depressed,and the time of milk collection; Multiple logistic regression analysis was used to identify the risk factors for delayed lactation initiation in patients; Spearman rank correlation analysis was used to investigate the correlation between age,first birth,pre pregnancy BMI,depression score,anxiety score,and delayed lactation initiation.【Results】There was no statistically significant difference (P>0.05) between the two groups in terms of whether insulin therapy was used,whether cesarean section was performed,preterm delivery,and milk collection time; The delayed group had significantly higher rates of age ≥ 35 years old,initial delivery,pre pregnancy BMI ≥ 30.0 kg/m2,excessive weight gain during pregnancy,and anxiety or depression compared to the non delayed group (P<0.05); Multivariate logistic regression analysis showed that age ≥ 35 years old,first birth,pre pregnancy BMI ≥30.0 kg/m2,excessive weight gain during pregnancy,depression or anxiety were the risk factors for delayed lactation in pregnant women with diabetes; Age,first birth,BMI before pregnancy,weight gain during pregnancy,depression or anxiety of pregnant women with diabetes were positively correlated with delayed lactation (P<0.05).【Conclusion】The risk factors of delayed lactation initiation in pregnant women with diabetes are elderly parturient,primipara,obesity,excessive weight gain during pregnancy,depression or anxiety.
