Abstract 【Objective】 To explore the differential value of magnetic resonance imaging (MRI) combined with computed tomography angiography (CTA) or CT for vascular Parkinsonism (VP) and idiopathic Parkinson's disease (PD). 【Methods】 The clinical data of 50 patients with VP (VP group) and 50 patients with PD (PD group) treated in Xuchang Wanrong Hospital of Traditional Chinese Medicine and Rehabilitation from May 2021 to May 2023 were retrospectively analyzed. All patients underwent MRI, CT or CTA examinations. The width of the midbrain-pontine sulcus, midbrain diameter, and the ratio of midbrain-pontine sulcus width to midbrain diameter were compared between the two groups. Lesions in the subcortical white matter, basal ganglia and thalamus were also compared. Receiver operating characteristic (ROC) curve was adopted to analyze the diagnostic performance of MRI combined with CT or CTA for identifying VP. 【Results】 The midbrain-pontine sulcus width and the ratio of midbrain-pontine sulcus width to midbrain diameter in the PD group were lower than those in the VP group, with statistically significant differences (all P<0.05). No significant difference was found in midbrain diameter between the two groups (P>0.05). The detection rates of lesions in subcortical white matter, basal ganglia and thalamus in the VP group were higher than those in the PD group, and the differences were statistically significant (all P<0.05). ROC curve analysis showed that the area under the curve (AUC) of MRI was 0.799 with a Youden index of 0.497; the AUC of CTA or CT was 0.834 with a Youden index of 0.615; the AUC of MRI combined with CTA or CT reached 0.951, which was significantly higher than that of MRI alone and CTA or CT alone (all P<0.05). 【Conclusion】 MRI combined with CTA or CT possesses high clinical value in the differential diagnosis of PD and VP, which is worthy of clinical promotion.
