Abstract 【Objective】 To investigate the predictive value of epicardial adipose tissue (EAT) thickness for chronic heart failure (CHF) in patients with type 2 diabetes mellitus (T2DM). 【Methods】 The clinical data of 120 patients with T2DM admitted to Anyang People's Hospital from May 2022 to May 2024 were retrospectively analyzed. Among them, 52 patients who developed CHF during follow-up were assigned to the observation group, and 68 patients without CHF were assigned to the control group. Glucose metabolic indicators [fasting plasma glucose (FPG), 2-hour postprandial plasma glucose (2hPG), glycated hemoglobin (HbA1c), fasting insulin (FINS)], EAT thickness and coronary artery disease burden (Gensini score) were compared between the two groups. Logistic regression was used to analyze the influencing factors for CHF in T2DM patients, and receiver operating characteristic (ROC) curve was adopted to evaluate the clinical predictive value of EAT thickness for CHF in T2DM patients. 【Results】 The levels of FPG, 2hPG and HbA1c in the observation group were higher, while FINS level was lower than those in the control group (all P<0.05). EAT thickness and Gensini score in the observation group were higher than those in the control group (all P<0.05). Logistic regression analysis demonstrated that FPG, FINS, EAT thickness and Gensini score were influencing factors for CHF in T2DM patients (all P<0.05). ROC curve analysis showed that the area under the curve of EAT thickness for predicting CHF in T2DM patients was 0.895 (95%CI:0.600-0.823), with a sensitivity of 88.76%, specificity of 88.24% and Youden index of 0.670. 【Conclusion】 EAT thickness presents favorable predictive value for CHF in T2DM patients and can be applied for clinical risk assessment.
