| Effects of Continuous Renal Replacement Therapy on Microinflammatory Status, Circulating Endothelial Cells, and Toll-Like Receptor 4 in Patients with Acute Renal Failure |

Abstract 【Objective】 To investigate the effects of continuous renal replacement therapy (CRRT) on microinflammatory status, circulating endothelial cells (CECs), and Toll-like receptor 4 (TLR4) in patients with acute renal failure (ARF). 【Methods】 A total of 122 patients with ARF admitted to our hospital from January 2022 to January 2025 were randomly divided into an observation group and a control group, with 61 cases in each group. The observation group received CRRT, while the control group was treated with intermittent hemodialysis (HD). The microinflammatory markers [C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)], renal function indices [blood urea nitrogen (BUN), serum creatinine (SCr)], levels of CECs and TLR4, and incidence of complications were compared between the two groups before treatment and after 2 weeks of treatment. 【Results】 After treatment, the levels of CRP, TNF-α, IL-6, BUN, SCr, CECs, and TLR4 in both groups were significantly lower than those before treatment, and the reductions were more pronounced in the observation group than in the control group (P<0.05). There was no statistically significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】 CRRT in patients with ARF can effectively alleviate the microinflammatory state, improve renal function, and reduce the levels of CECs and TLR4, with a favorable safety profile.
