													     		                            						                            																	    Abstract  【Objective】To explore the predictive value of preoperative serum lipoprotein associated phospholipase A2 (Lp-PLA2) and nucleotide binding oligomeric domain like receptor protein 3 (NLRP3) levels for cardiovascular adverse events (MACE) in high-risk coronary heart disease patients undergoing intra-aortic balloon counterpulsation (IABP) assisted percutaneous coronary intervention (PCI).【Methods】A total of 120 high-risk coronary heart disease patients admitted to our hospital were selected and all underwent IABP assisted PCI treatment. The incidence of MACE within 6 months after surgery was statistically analyzed and divided into the non MACE group and the MACE group. The preoperative serum Lp-PLA2 levels were compared between the two groups NLRP3 level. The correlation between preoperative serum Lp-PLA2 and NLRP3 levels and the degree of coronary stenosis were analyzed. The receiver operating characteristic (ROC) curve was used to analyze the value of preoperative serum Lp-PLA2 and NLRP3 level in predicting postoperative MACE.【Results】The incidence of MACE in 120 high-risk coronary heart disease patients after IABP assisted PCI was 34.17% (41/120); Preoperative serum Lp-PLA2 in the MACE group The levels of NLRP3 were higher than those in the non MACE group (P<0.05). Spearman correlation analysis showed that preoperative serum Lp-PLA2 The level of NLRP3 was positively correlated with the degree of coronary stenosis (rs=0.750, 0.815, all P<0.05). The level of preoperative serum Lp-PLA2 and NLRP3 in severe patients was higher those in moderate and mild patients, with moderate patients having higher levels than mild patients (P<0.05). ROC curve analysis showed that the area under the curve predicted by preoperative serum Lp-PLA2 and NLRP3 level alone was 0.770 and 0.844, respectively. The joint prediction of AUC by the two was 0.909 (P<0.05).【Conclusion】The level of Preoperative serum Lp-PLA2 and NLRP3 was positively correlated with the degree of coronary artery stenosis in high-risk coronary heart disease patients. The combined detection of the two can provide a certain reference for clinical prediction of MACE in high-risk coronary heart disease patients after IABP assisted PCI surgery.
