Abstract  【Objective】 To explore the effects of Qingxuan Jieyu Decoction on blood pressure and levels of homocysteine (Hcy) and vascular endothelial growth factor (VEGF) in elderly patients with hypertension and vertigo. 【Methods】 A total of 102 elderly patients with hypertension and vertigo treated at two hospital were randomly divided into an observation group (Qingxuan Jieyu Decoction + Western medicine treatment) and a control group (Western medicine treatment), with 51 patients in each group. The therapeutic effect, adverse reactions, traditional Chinese medicine (TCM) syndrome scores, systolic blood pressure (SBP), diastolic blood pressure (DBP), dizziness assessment rating scale (DARS) scores, and levels of Hcy and VEGF before and after treatment were compared in both groups. 【Results】 The total effective rate in the observation group was 88.24%, which was significantly higher than 70.59% in the control group (P<0.05). After treatment, scores for vertigo, headache, chest tightness, irritability, palpitations, and insomnia in both groups were significantly lower than before treatment, with the observation group showing even lower scores than the control group (P<0.05). After treatment, SBP, DBP, and DARS scores in both groups were significantly lower than before treatment (P<0.05), and the observation group showed even lower scores of SBP, DBP, and DARS than the control group (P<0.05). After treatment, levels of Hcy and VEGF in both groups were significantly lower than before treatment, while levels of Hcy and VEGF in the observation group were lower than those in the control group (P<0.05). The total incidence of adverse reactions in both groups showed no significant difference (P>0.05). 【Conclusion】 Qingxuan Jieyu Decoction can effectively alleviate vertigo symptoms in elderly patients with hypertension and vertigo, control blood pressure levels, protect vascular endothelial function, and has few adverse reactions.
