Effect of Hemodialysis with Different Fluxes on Cardiac Function, Myocardial Enzymes and prognosis in Elderly Patients with Diabetic Nephropathy

Abstract  【Objective】To investigate the effects of hemodialysis with different fluxes on cardiac function, myocardial enzymes and prognosis in elderly patients with diabetic nephropathy (DN). 【Methods】 The clinical data of 88 elderly patients with DN admitted to the hospital were retrospectively analyzed. There were 46 patients treated with high-throughput hemodialysis in group A and in the same period 42 patients were treated with low-throughput hemodialysis in group B. Cardiac function indicators: left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD), interventricular septal diastolic thickness (IVSDT), Myocardial enzyme indicators such as creatine phosphokinase (CK), lactate dehydrogenase (LDH), hydroxybutyrate dehydrogenase (HBDH) and blood pressure were compared before and after treatment in two groups. The incidence of cardiovascular events in the 12 months of follow-up were statistically analyzed. The survival at 6 months and 12 months during the follow-up period were counted. 【Results】There was no significant difference in each indicator between the two groups before treatment (P>0.05). LVEF level was increased after treatment in both groups (P<0.05), and LVEF level in group A was significantly higher than that in group B (P<0.05). After treatment, LVEDD and IVSDT in both groups were reduced (P<0.05), and levels of LVEDD and IVSDT in group A were significantly lower than those in group B (P<0.05). CK, LDH and HBDH levels in the two groups were all decreased after treatment (P<0.05), while levels of CK, LDH and HBDH in group A were significantly lower than those in group B (P<0.05). There was no significant difference in systolic and diastolic blood pressure between the two groups before and after treatment (P>0.05). The total incidence rate of cardiovascular events in group A was lower than that in group B (P<0.05). There was no significant difference in 6-month survival rate between the two groups(P>0.05), while the 12-month survival rate in group A was higher than that in group B (P<0.05). 【Conclusion】High-throughput hemodialysis for elderly patients with DN can effectively improve their cardiac function, reduce the level of myocardial enzymes and the incidence of cardiovascular events, and improve the survival rate.
