| Relationship of Serum Endothelin, Leptin and Vaspin Levels with Insulin Resistance in Gestational Diabetes Mellitus Patients |

Abstract 【Objective】To investigate the correlation of serum endothelin, leptin and vaspin (visceral adipose tissue-derived serine protease inhibitor) with insulin resistance in gestational diabetes mellitus. 【Methods】A total of 352 pregnant women who had regular prenatal examination in the Affiliated Hospital of Yan'an University from June 2014 to June 2019 were selected in the study. According to the results of oral 75 g glucose tolerance test (OGTT), patients were divided into the gestational diabetes mellitus group (GDM, n=126) and the control group with non-GDM (n=126) by. Serum endothelin level was measured by radioimmunoassay, serum leptin and vaspin levels were detected by enzyme-linked immunosorbent assay (ELISA), blood glucose and lipid routine indices were measured and the insulin resistance index (HOMA-IR) was calculated. The relationship of endothelin, leptin and vaspin with blood glucose and lipid was analyzed. 【Results】The levels of systolic blood pressure(SBP), fasting blood glucose(FPG), glycosylated hemoglobin(HbAlc), triacylglycerol(TG), low-density lipoprotein cholesterol(LDL-C), fasting insulin(FINS), HOMA-IR, serum endothelin, leptin and vaspin in gestational diabetes mellitus group were significantly higher than those in the control group (all P<0.05). Serum endothelin level in gestational diabetes mellitus patients was positively correlated with FPG, HbAlc, FINS and HOMA-IR levels (P<0.05); Leptin level was positively correlated with FPG, TG, HbAlc, FINS and HOMA-IR levels (P<0.05); And vaspin level was positively correlated with FPG, TG, FINS and HOMA-IR levels (P<0.05). High levels of FINS, endothelin, HOMA-IR, leptin and vaspin were risk factors for gestational diabetes mellitus (all P<0.05). 【Conclusion】The levels of HOMA-IR, serum endothelin, leptin and vaspin in patients with gestational diabetes mellitus are significantly higher than those in the control group. Levels of serum endothelin, leptin and vaspin are positively correlated with the level of HOMA-IR, which may affect the occurrence and development of gestational diabetes mellitus together.
