Complications and Influencing Factors of TEP、TAPP and Open Tension-free Hernia Repair for Adult Inguinal Hernia

Abstract  【Objective】To investigate the complications and influencing factors of three hernia repair methods: total extraperitoneal laparoscopic (TEP), laparoscopic transabdominal preperitoneal (TAPP) and open tension-free hernia repair for the treatment of adult inguinal hernia.【Methods】From March 2015 to March 2019, 150 cases of adult inguinal hernia admitted to our general surgery department were analyzed retrospectively. According to different surgical methods, they were divided into three groups: A, B and C. There were 50 cases in each group. Among them, group A was treated with TEP (56 sides), group B was treated with TAPP (58 sides), and group C was treated with open tension-free hernia repair (54 sides). The incidence of postoperative complications was compared among the three groups, and the influencing factors of possible complications were observed and analyzed. 【Results】The incidence of complications was 8.00% (4 cases) in group A, 10.00% (5 cases) in group B, and 26.00%(13 cases) in group C; The difference between the groups was statistically significant (P<0.05).The results of univariate analysis showed that the occurrence of postoperative complications of inguinal hernia was related to the operation mode, BMI and the degree of hernia ring adhesion (P<0.05),while it was not related to gender, age, size of hernia sac, location of hernia, type of hernia and internal diseases (P>0.05) . Multivariate analysis showed that operation mode, BMI and degree of hernial ring adhesion were independent factors of postoperative complications of inguinal hernia (OR=2.16, 1.89, 2.14, P<0.05). 【Conclusion】The operative method, BMI and the degree of hernial ring adhesion were independent risk factors of postoperative complications in patients with inguinal hernia. Therefore, in order to reduce the incidence of postoperative complications, we should select the appropriate operation mode according to the patients' own situation, strengthen the control of BMI and the use of hernia bag more standardized.
