Effect of Serum Uric Acid Level on the Risk of Diabetic Nephropathy in Patients with Diabetic Retinopathy

Abstract  【Objective】 To investigate the effect of serum uric acid level on the risk of diabetic nephropathy (DN) in patients with type 2 diabetic retinopathy (DR). 【Methods】 A total of 100 patients with type 2 diabetes mellitus without diabetic retinopathy in our hospital were enrolled in the study. Serum uric acid level was measured. The patients were followed up for incidence of DR and DN. The correlation between serum uric acid and DN was evaluated and the influencing factors of DN in patients with type 2 diabetes DR were analyzed. 【Results】 The average follow-up time was (33.2±3.1) months. No patients were lost to follow-up. A total of 23 cases (23.0%) of DN occurred, and the no-DN rate was 87.0% (87/100) in 2 years. The uric acid level in patients with DN was higher than that in patients without DN (P<0.05). Kaplan-Meier and Log-rank test showed that patients with elevated blood uric acid levels had shorter DN accumulation time than patients with reduced serum uric acid levels (P<0.05). Cox analysis showed that age, duration of diabetes, carotid atherosclerotic plaque, fundus lesions, fasting blood glucose, and uric acid were independent factors affecting DN in patients with type 2 diabetes (P<0.05). ROC analysis showed that the area under the curve uric acid to predict DN in DR patients with type 2 diabetes was 0.74, the sensitivity was 65.2% and the specificity was 78.9%.【Conclusion】 The risk of DN is significantly increased in type 2 diabetic DR patients with elevated serum uric acid level, and serum uric acid is an effective biomarker for predicting DN in patients with type 2 diabetes.
