经口腔前庭入路治疗分化型甲状腺癌的临床疗效及对患者喉返神经功能的影响

Abstract 【Objective】To investigate the clinical efficacy of the transoral vestibular approach in the treatment of differentiated thyroid cancer and its impact on recurrent laryngeal nerve function in patients. 【Methods】A retrospective analysis was conducted on the clinical data of 106 patients with differentiated thyroid cancer treated in our hospital from January 2022 to December 2023.The patients were divided into a control group(underwent traditional open surgery)and an observation group(underwent transoral vestibular approach endoscopic surgery),with 53 cases in each group. The swallowing function, pain level, and incision aesthetic satisfaction of the patients were assessed using the Swallowing Impairment Score(SIS),Visual Analog Scale(VAS),and Patient and Observer Scar Assessment Scale.The clinical efficacy, perioperative indicators, recurrent laryngeal nerve injury, and incidence of complications were compared between the two groups. 【Results】There was no significant difference in the clinical efficacy rate and the total incidence of postoperative complications between the two groups(both P>0.05).The observation group had a longer operation time than the control group,but the intraoperative blood loss and postoperative drainage volume were lower than those in the control group, with significant differences(P<0.05).There was no significant difference in the number of lymph nodes dissected between the two groups(P>0.05).The SIS, VAS, Observer Scar Assessment Scale(OSAS) and Patient Scar Assessment Scale(PSAS) scores in the observation group were lower than those in the control group, and the serum C-reactive protein(CRP) and interleukin-6(IL-6) levels were also lower than those in the control group, with significant differences(P<0.05).The incidence of temporary hoarseness in the observation group was lower than that in the control group, with a significant difference(P<0.05).No permanent hoarseness occurred in either group. 【Conclusion】The transoral vestibular approach endoscopic surgery shows good clinical efficacy in the treatment of differentiated thyroid cancer. It has significant advantages in improving patients' swallowing function, reducing postoperative pain, enhancing incision aesthetic satisfaction, and protecting the recurrent laryngeal nerve, with good safety.
