Efficacy of Nail-Rod Reverse Arch Folding Technique in Elderly Thoracolumbar Burst Fractures |

Abstract 【Objective】To investigate the effect of nail-rod reverse arch folding technique in elderly patients with thoracolumbar burst fractures. 【Methods】A total of 102 elderly patients with thoracolumbar burst fracture, who diagnosed and treated in our hospital from May 2020 to May 2023, were divided into the observation group and the control group by random number table method, with 51 cases in each group. The control group was treated with posterior decompression and internal fixation with bone grafting, while the observation group was treated with posterior decompression and internal fixation with bone grafting plus internal fixation with nail-rod reverse arch folding top. Follow-up was performed up to 6 months after surgery. The operation, healing time, pain, lumbar function, imaging parameters, spinal canal invasion and complications were compared between the two groups. 【Results】The operation time of the observation group was longer than that of the control group, the amount of intraoperative blood loss was more than that of the control group, but the fracture healing time was shorter than that of the control group, and the differences were statistically significant (P<0.05). The visual analogue scale (VAS) scores of the observation group were lower than those of the control group at 1 month and 3 months after surgery (P<0.05). The differences of Japanese Orthopaedic Association (JOA) score and Oswestry Disability Index (ODI) before and after treatment in the observation group were higher than those in the control group (P<0.05). The ratio of anterior vertebrae edge height, the ratio of posterior vertebrae edge height and the Cobb Angle difference of the injured vertebra before and after treatment in the observation group were higher than those in the control group (P<0.05). Compared to pre-operation, the invasion rates of vertebral canal in both groups decreased at 1 month and 6 months after operation. Compared with 1 month after operation, the invasion rates of vertebral canal in both groups at 6 months after operation were lower than those at 1 month after operation. At 1 month and 6 months after operation, the invasion rates of vertebral canal in observation group were lower than those in control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】The application of napping technique in elderly thoracolumbar burst fractures is remarkable, which can promote fracture healing, improve the function of lumbar spine and obtain satisfactory correction results.
