Application of PET/CT and HRCT in the Differential Diagnosis of Peripheral Lung Cancer |

Abstract 【Objective】 To investigate the application of PET/CT and HRCT in the differential diagnosis of peripheral lung cancer. 【Methods】 A retrospective analysis of 152 patients with suspected peripheral lung cancer from February 2014 to March 2017 was performed. All patients underwent PCT/CT and HRCT examination after admission, and were confirmed by histology, puncture or addition. The number and size of the lesions were determined, the maximum standardized intake of the lesion area was calculated, and the area of HRCT,PET/CT and their combination were analyzed under the receiver operating curve (ROC) of the subjects for the diagnosis of peripheral lung cancer.【Results】 (1) 24 cases had lesions of 0.5~1 cm in diameter, 22 cases had clear signs in PET/CT and 19 cases had clear signs in HRCT; (2) 55 cases had lesion diameter of 1~2 cm, 53 cases had clear signs in PET/CT and 55 cases had clear signs in HRCT; (3) 36 cases had lesion diameter of 2~3 cm, 38 cases had clear signs in PET/CT and 34 cases had clear signs in HRCT; (4) the standardized maximum uptake of peripheral lung cancer was significantly higher than that of non-peripheral lung cancer (P<0.05); (5) 106 cases were diagnosed as peripheral lung cancer by PET/CT, and 1 case was negative. In 9 cases, the sensitivity, specificity and accuracy of diagnosing peripheral lung cancer were 84.80%, 70.37% and 82.23%, respectively; (6) HRCT was used to diagnose 93 cases of peripheral lung cancer and 32 cases of negative lung cancer. The sensitivity, specificity and accuracy of diagnosing peripheral lung cancer were 74.40%, 81.48% and 75.65%, respectively; (7) PET/CT+HRCT was used to diagnose 113 cases of peripheral lung cancer, 12 cases of negative lung cancer and 12 cases of peripheral lung cancer. The sensitivity, specificity and accuracy were 90.40%, 88.89% and 90.13% respectively. 【Conclusion】 The combination of HRCT and PET/CT can significantly reduce the misdiagnosis rate of peripheral lung cancer, and significantly improve the sensitivity, specificity and accuracy of the diagnosis of peripheral lung cancer.
