Abstract：【Objective】To investigate the therapeutic effect of Huotan Xijing Tang combined with thrombolysis on patients with acute cerebral infarction and its impact on cognitive function and cerebral hemodynamics. 【Methods】A total of 108 patients with acute cerebral infarction were randomly divided into an observation group and a control group,with 54 cases in each group. The control group was treated with thrombolysis,while the observation group received Huotan Xijing Tang combined with thrombolysis. The therapeutic effects,adverse reactions,and differences in related indicators before and after treatment were compared between the two groups. 【Results】The overall effectiveness rate of the observation group was 94.44%(51/54),which was significantly higher than the 81.48%(44/54) of the control group,and the difference was statistically significant(P<0.05). After treatment,the scores for Traditional Chinese Medicine(TCM) symptoms such as sudden fainting,hemiplegia,and deviation of the mouth and tongue were lower in both groups compared to before treatment(P<0.05). Moreover,the observation group had a greater reduction in these scores than the control group(P<0.05). After treatment,the National Institutes of Health Stroke Scale(NIHSS) scores in both groups significantly reduced,and the Montreal Cognitive Assessment(MoCA) scores and Activities of Daily Living(ADL) scores increased compared to before treatment(P<0.05). The improvement in scores of the observation group was better than that of the control group(P<0.05). After treatment,the pulsatility index(PI) levels in both groups were lower than before treatment,and the peak flow velocity(VP) and mean flow velocity(Vm) levels of the bilateral middle cerebral artery were higher than before treatment(P<0.05). The improvement in these parameters of the observation group was better than that of the control group(P<0.05). The overall incidence of adverse reactions between the two groups showed no statistically significant difference(P>0.05). 【Conclusion】Huotan Xijing Tang combined with thrombolysis in patients with acute cerebral infarction can significantly improve therapeutic effects,reduce TCM symptom scores,enhance neural function,improve cerebral hemodynamics,and elevate patients' cognitive function and daily living ability with minimal drug adverse reactions.
