黄体酮联合宫腔镜刨削系统治疗子宫内膜息肉不孕症患者的临床疗效

摘要【目的】探讨黄体酮联合宫腔镜刨削系统(IBS)治疗子宫内膜息肉不孕症患者的临床效果。【方法】选取2019年8月至2021年6月在西安市第四医院诊治的92例子宫内膜息肉不孕症患者,采用随机数字表法分为对照组(采用黄体酮联合普通双极宫腔镜电切系统治疗)和观察组(采用黄体酮联合宫腔镜IBS治疗),每组46例。比较两组患者手术时间、灌注介质总量及灌注介质吸收量、临床疗效、子宫内膜息肉复发情况及妊娠情况及治疗前后月经周期、月经量及子宫内膜厚度。【结果】观察组手术时间短于对照组(P<0.05),灌注介质总量和灌注介质吸收量均低于对照组(P<0.05)。治疗前,两组患者月经周期、月经量及子宫内膜厚度比较,差异均无统计学意义(P>0.05);治疗后,两组患者月经周期、月经量及子宫内膜厚度均少于治疗前(P<0.05),且观察组月经周期、月经量及子宫内膜厚度均少于对照组(P<0.05)。观察组总有效率高于对照组,复发率低于对照组,妊娠率高于对照组,其差异均有统计学意义(P<0.05)。【结论】黄体酮联合宫腔镜IBS治疗子宫内膜息肉不孕症可缩短手术时间,减少灌注介质总量及吸收量,调节月经周期和月经量,降低子宫内膜厚度和复发率,提高临床疗效和妊娠率,值得临床推广应用。

Abstract：【Objective】To investigate the clinical effect of progesterone combined with hysteroscopic planing system (IBS) in the treatment of infertility patients with endometrial polyps. 【Methods】A total of 92 infertile patients with endometrial polyps diagnosed and treated in Xi'an Fourth Hospital from August 2019 to June 2021 were randomly divided into control group (treated with progesterone combined with common bipolar hysteroscopic electrotomy system) and observation group (treated with progesterone combined with hysteroscopic IBS),with 46 cases in each group. The operation time,total amount and absorption of perfusion media,clinical efficacy,recurrence of endometrial polyps,pregnancy,menstrual cycle,menstrual volume and endometrial thickness before and after treatment were compared between the two groups. 【Results】The operation time in the observation group was shorter than that in the control group (P<0.05). The total amount of perfusion medium and the absorption of perfusion medium in the control group were lower than those in the control group (P<0.05). Before treatment,there was no significant difference in menstrual cycle,menstrual volume and endometrial thickness between the two groups (P>0.05); After treatment,the menstrual cycle,menstrual volume and endometrial thickness of the two groups were less than those before treatment (P<0.05),and the menstrual cycle,menstrual volume and endometrial thickness of the observation group were less than those of the control group (P<0.05). The total effective rate of the observation group was higher than that of the control group,the recurrence rate was lower than that of the control group,and the pregnancy rate was higher than that of the control group. The difference was statistically significant (P<0.05). 【Conclusion】Progesterone combined with hysteroscopic IBS in the treatment of endometrial polyp infertility can shorten the operation time,reduce the total amount and absorption of perfusion media,regulate the menstrual cycle and menstrual volume,reduce the endometrial thickness and recurrence rate,and improve the clinical efficacy and pregnancy rate. It is worthy of clinical application.
