慢性萎缩性胃炎患者血清维生素B<sub>12</sub>水平与周围神经病变的相关性分析

摘要 【目的】探讨慢性萎缩性胃炎患者血清维生素B₁₂水平与周围神经病变的相关性。【方法】选取2015年6月至2018年6月本院收治的375例慢性萎缩性胃炎患者,以患者血清维生素B₁₂中位数为界将患者分为低水平组(血清维生素B₁₂<157.70 pg/mL,n=89)和高水平组(血清维生素B₁₂≥157.70 pg/mL,n=286)。比较两组患者周围神经传导速度,并分析维生素B₁₂和周围神经传导速度的相关性。采用单因素及多因素Logistic回归分析影响慢性萎缩性胃炎患者发生周围神经病变的相关因素。【结果】低水平组患者正中神经、尺神经、胫骨神经、腓肠神经传导速度明显低于高水平组患者,差异有统计学意(P<0.05)。Pearson相关性分析显示,慢性萎缩性胃炎患者维生素B₁₂水平与正中神经、尺神经、胫骨神经、腓肠神经传导速度均呈正相关性(P<0.05)。多因素Logistic回归分析结果显示,幽门螺杆菌、同型半胱氨酸、神经生长因子、神经营养因子、维生素B₉及维生素B₁₂与慢性萎缩性胃炎患者周围神经病变关系密切(P<0.05)。【结论】慢性萎缩性胃炎患者血清维生素B₁₂水平与周围神经传导速度关系密切,且为周围神经病变的独立保护因素。
