Comparison of Effect and Safety between Modified Prone with Leg Separation and Traditional Prone Position after Lithotomy in Treatment of Complex Kidney Stones

Abstract:【Objective】 To compare the effect and safety of modified prone posture with leg separation and traditional prone posture after lithotomy in the treatment of complex kidney stones. 【Methods】 A total of 120 patients with complex kidney stones treated in our hospital were randomly divided into the observation group (n=60) and the control group (n=60). Patients in the observation group were in the modified prone with leg separation to receive percutaneous nephrolithotomy(PCNL); while patients in the group were in the conventional prone after lithotomy for PCNL. The surgical efficacy and complications were compared between the two groups. 【Results】 The operation time and channel establishment time of the observation group were shorter than those of the control group; and the intraoperative blood loss in the observation group was lower than that in the control group (P<0.05). In the observation group, the hospitalization time and the first exhaust time were shorter, the reoperation rate was lower, and the rate of stone removal was higher than in the control group (P<0.05). The VAS score of the observation group was lower than that of the control group after surgery (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). The quality of life score in the observation group at 4 weeks after operation was higher than that in the control group; and the difference was statistically significant (P<0.05). 【Conclusion】For patients with complicated renal calculi, the use of modified prone posture with leg separation during percutaneous nephrolithotripsy can shorten the operation time, increase the rate of stone removal, promote postoperative recovery, and improve the quality of life without increasing complications.
