T淋巴细胞及炎症因子水平变化与甲亢性肝损害相关性分析

摘要 目的】 探讨CD4+T细胞、CD4+CD25+调节性T细胞及炎症因子与甲亢性肝功能损伤的相关性。【方法】 选取2015年1月至2018年3月本院收治的132例甲亢患者的临床资料,其中伴有肝功能损伤患者70例(观察组),肝功能正常患者62例(对照组)。检测比较两组患者CD4+T细胞和CD4+CD25+调节性T细胞水平。比较两组患者血清白细胞介素6(IL-6)、白细胞介素10(IL-10)、促甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、四碘甲状腺原氨酸(T4)水平。【结果】 观察组T3、T4水平明显高于对照组,差异有统计学意义(P<0.05);观察组与对照组TSH水平比较,差异无统计学意义(P>0.05)。观察组血清IL-6、IL-10水平明显高于对照组,差异有统计学意义(P<0.05);观察组CD4+T细胞和CD4+CD25+调节性T细胞水平明显低于对照组,差异有统计学意义(P<0.05)。肝功损害患者ALT为(104.38±32.10)U/L,AST为(90.03±24.39)U/L,ALP为(82.23±17.44)U/L;将患者T淋巴细胞、炎症因子与ALT、AST、ALP及甲状腺激素水平进行Pearson相关分析,结果显示,CD4+CD25+调节性T细胞与ALT、AST、ALP、T3和T4均呈负相关(r=-0.544、-0.434、-0.414、-0.322、-0.303,均P<0.05)。【结论】 甲亢性肝功能损伤患者中血清IL-6、IL-10水平明显上升,而CD4+T细胞和CD4+CD25+调节性T细胞水平明显下降,同时T淋巴细胞亚群的改变与患者的肝功能及甲状腺激素水平均密切相关。

Abstract:【Objective】 To investigate the correlation between CD4+ T cells, CD4+ CD25+ regulatory T cells, inflammatory factors and hyperthyroidism liver function injury.【Methods】 The clinical data of 132 patients with hyperthyroidism in our hospital from January 2015 to March 2018 were selected, including 70 patients with liver function injury (observation group) and 62 patients with normal liver function (control group). The levels of CD4+ T cells and CD4+ CD25+ regulatory T cells were detected and compared between the two groups. The serum levels of interleukin-6 (IL-6), interleukin-10 (IL-10), thyroid stimulating hormone (TSH), triiodothyronine (T3) and tetraiodothyronine (T4) were compared between the two groups.【Results】 The level of T3 and T4 in the observation group was significantly higher than that in the control group (P<0.05); there was no significant difference in TSH level between the observation group and the control group (P>0.05). The levels of serum IL-6 and IL-10 in the observation group were significantly higher than those in the control group (P<0.05); the levels of CD4+ T cells and CD4+ CD25+ regulatory T cells in the observation group were significantly lower than those in the control group (P<0.05). ALT and AST were (104.38±32.10) U/L and (90.03±24.39) U/L, respectively, ALP was (82.2±17.44) U/L; Pearson correlation analysis of T lymphocytes, inflammatory factors and ALT, AST, ALP and thyroid hormone levels showed that CD4+ CD25+ regulatory T cells were negatively correlated with ALT, AST, ALP, T3 and T4 (r=-0.544, -0.434,-0.414, -0.322, -0.303, all P<0.05).【Conclusion】 The levels of serum IL-6 and IL-10 increased significantly, while the levels of CD4+ T cells and CD4+ CD25+ regulatory T cells decreased significantly in patients with hyperthyroid liver function injury. Meanwhile, the changes of T lymphocyte subsets are closely related to liver function and thyroid hormone levels.
