心理干预联合中药熏洗对PPH术后患者应激反应及感觉功能障碍的影响

摘要【目的】探讨心理干预联合中药熏洗对吻合器痔上黏膜环切术(PPH)患者术后应激反应及感觉功能障碍的影响。【方法】选择2015年1月至2016年6月本院收治的PPH术后感觉功能障碍患者124例为研究对象,采用随机数表法分为观察组和对照组各62例,所有患者均给予常规抗生素预防感染及止痛治疗,对照组联合应用中药熏洗,观察组在对照组的基础上联合应用心理干预。比较两组生理应激反应、感觉功能障碍评分、临床疗效等指标。【结果】观察组治疗有效率为91.94%明显高于对照组的75.81%(χ²=5.962, P<0.05);血清5-羟色胺(5-HT)、前列腺素E₂(PGE₂)、组胺(HA)含量明显低于对照组(P<0.05);疼痛、坠胀、排尿障碍评分明显低于对照组(P<0.05)。【结论】心理干预联合中药熏洗有助于缓解PPH术患者生理应激反应,改善术后感觉功能障碍,提高临床疗效。

Abstract：【Objective】To investigate the effect of psychological intervention in combination with herbal fumigation on stress reaction and sensory disturbance of patients undergoing procedures for prolapsed hemorrhoids (PPH).【Methods】A total of 124 cases of patients with sensory disturbance after PPH who were admitted to our hospital from January 2015 to June 2016 were selected as the research subjects and divided into the observation group and the control group using the random number table method, with 62 cases in each group. All the patients received conventional antibiotics for infection prevention and pain-killing treatment. Patients in the control group were also given traditional Chinese herbal fumigation, while patients in the observation group received psychological intervention on the basis of the control group. The scores of physiological stress reaction and sensory disturbance, clinical efficacy and other indicators between the two groups were compared.【Results】The effective rate in the observation group was 91.94%, which was significantly higher than that of 75.81% in the control group ( P < 0.05). The levels of serum 5-hydroxytryptamine (5-HT), prostaglandin E₂ (PGE₂) and histamine (HA) were significantly lower than those in control group ( P < 0.05). The scores of pain, fall-swell and urination disorder were significantly lower than those in control group as well ( P < 0.01).【Conclusion】Psychological intervention in combination with herbal fumigation can help to relieve the physiological stress reaction of patients undergone PPH, improve the postoperative sensory disturbance, and enhance the clinical efficacy.
