PET/CT与HRCT在周围型肺癌鉴别诊断中的应用

摘要 【目的】 探讨正电子发射计算机断层显像(PET)/CT与高分辨率CT(HRCT)在周围型肺癌鉴别诊断中的应用。【方法】 对临床疑似周围型肺癌的152例患者均行PCT/CT及HRCT检查,并经组织学确诊。对病灶的数目、大小等进行判断。采用受试者工作曲线(ROC)下面积分析HRCT、PET/CT及两者联合对周围型肺癌诊断价值。【结果】 ①152例患者共有125例诊断为周围型肺癌,病灶直径0.5~1 cm者24例,PET/CT和HRCT中征相清楚者分别为22例和19例;②直径1~2 cm 55例,PET/CT和HRCT中征相清楚者分别为53例和55例;③直径2~3 cm者46例,PET/CT和HRCT中征相清楚者分别为38例和34例;④周围型肺癌其标准化最大摄取量明显高于非周围型肺癌(P<0.05);⑤PET/CT诊断为周围型肺癌106例,阴性19例,敏感度、特异性、准确度分别为84.80%、70.37%、82.23%;⑥HRCT诊断为周围型肺癌93例,阴性32例,敏感度、特异性、准确度分别为74.40%、81.48%、75.65%;⑦PET/CT+HRCT检查诊断为周围型肺癌113例,阴性12例,敏感度、特异性、准确度分别为90.40%、88.89%、90.13%。【结论】 HRCT与PET/CT结合能显著降低周围型肺癌诊断的误诊率,明显提高周围型肺癌诊断的敏感度、特异性及准确度。

Abstract【Objective】 To investigate the application of PET/CT and HRCT in the differential diagnosis of peripheral lung cancer. 【Methods】 A retrospective analysis of 152 patients with suspected peripheral lung cancer from February 2014 to March 2017 was performed. All patients underwent PCT/CT and HRCT examination after admission, and were confirmed by histology, puncture or addition. The number and size of the lesions were determined, the maximum standardized intake of the lesion area was calculated, and the area of HRCT,PET/CT and their combination were analyzed under the receiver operating curve (ROC) of the subjects for the diagnosis of peripheral lung cancer.【Results】 (1) 24 cases had lesions of 0.5~1 cm in diameter, 22 cases had clear signs in PET/CT and 19 cases had clear signs in HRCT; (2) 55 cases had lesion diameter of 1~2 cm, 53 cases had clear signs in PET/CT and 55 cases had clear signs in HRCT; (3) 36 cases had lesion diameter of 2~3 cm, 38 cases had clear signs in PET/CT and 34 cases had clear signs in HRCT; (4) the standardized maximum uptake of peripheral lung cancer was significantly higher than that of non-peripheral lung cancer (P<0.05); (5) 106 cases were diagnosed as peripheral lung cancer by PET/CT, and 1 case was negative. In 9 cases, the sensitivity, specificity and accuracy of diagnosing peripheral lung cancer were 84.80%, 70.37% and 82.23%, respectively; (6) HRCT was used to diagnose 93 cases of peripheral lung cancer and 32 cases of negative lung cancer. The sensitivity, specificity and accuracy of diagnosing peripheral lung cancer were 74.40%, 81.48% and 75.65%, respectively; (7) PET/CT+HRCT was used to diagnose 113 cases of peripheral lung cancer, 12 cases of negative lung cancer and 12 cases of peripheral lung cancer. The sensitivity, specificity and accuracy were 90.40%, 88.89% and 90.13% respectively. 【Conclusion】 The combination of HRCT and PET/CT can significantly reduce the misdiagnosis rate of peripheral lung cancer, and significantly improve the sensitivity, specificity and accuracy of the diagnosis of peripheral lung cancer.
