Value of Combined Detection of Epstein-Barr Virus Antibodies, EBV-DNA Load and Lymphocyte Subsets in the Diagnosis of Infectious Mononucleosis in Children

Abstract:【Objective】 To investigate the value of combined detection of Epstein-Barr virus (EBV) antibodies, EBV-DNA load, and lymphocyte subsets in the diagnosis of infectious mononucleosis (IM) in children. 【Methods】 A total of 82 children with IM admitted to our hospital were enrolled as the observation group, while 80 healthy children undergoing routine physical examinations during the same period were included as the control group. The levels of EBV antibodies, EBV-DNA load, and lymphocyte subsets were compared between the two groups. Receiver operating characteristic (ROC) curves were constructed to evaluate the diagnostic value of EBV antibodies, EBV-DNA load, and lymphocyte subsets, alone and in combination, for IM in children. 【Results】 The positive rates of EBV capsid antigen immunoglobulin G (EBV-CA-IgG), EBV capsid antigen immunoglobulin M (EBV-CA-IgM), and EBV-DNA load in the observation group were significantly higher than those in the control group (P<0.05). The levels of CD3+ and CD8+ cells were significantly higher, whereas CD4+ cells and the CD4+/CD8+ ratio were significantly lower in the observation group than in the control group (P<0.05). ROC analysis showed that the combined detection of EBV antibodies, EBV-DNA load, and lymphocyte subsets achieved a sensitivity of 93.9%, a specificity of 95.0%, and an area under the curve (AUC) of 0.985 for diagnosing IM in children. 【Conclusion】 Combined detection of EBV antibodies, EBV-DNA load, and lymphocyte subsets can improve the diagnostic accuracy of infectious mononucleosis in children and has high clinical application value.
