氨氯地平联合阿托伐他汀钙片治疗老年高血压合并冠心病患者的疗效及对血脂及心功能的影响

摘要【目的】探讨氨氯地平联合阿托伐他汀钙片治疗老年高血压合并冠心病患者的临床疗效及对血脂及心功能的影响。【方法】选择2022年9月至2024年9月漯河市舞阳县侯集镇卫生院收治的68例老年高血压合并冠心病患者,采用随机数字表法将患者分为对照组和观察组,每组34例。比较两组治疗效果、血压、血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]及心功能指标[左室射血分数(LVEF)、左心室舒张末内径(LVEDD)、左心室收缩末内径(LVESD)]。【结果】观察组临床有效率为94.12%(32/34),高于对照组的73.53%(25/34),差异有统计学意义(χ²=5.314,P<0.05)。治疗后,两组收缩压、舒张压低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清TC、TG、LDL-C水平均低于治疗前,HDL-C水平高于治疗前,且观察组血清TC、TG、LDL-C水平均低于对照组,HDL-C水平高于对照组,差异有统计学意义(P<0.05)。治疗后,两组LVEF高于治疗前,LVESD、LVEDD短于治疗前,且观察组LVEF高于对照组,LVESD、LVEDD短于对照组,差异有统计学意义(P<0.05)。【结论】氨氯地平联合阿托伐他汀钙片可有效改善老年高血压合并冠心病患者的血压、血脂及心功能,有良好的临床应用价值。

Abstract：【Objective】To explore the effect of amlodipine combined with atorvastatin calcium tablets on clinical efficacy, blood lipids and cardiac function in elderly patients with hypertension complicated by coronary heart disease. 【Methods】A total of 68 elderly patients with hypertension complicated by coronary heart disease admitted to Houji Town Health Center of Wuyang County, Luohe City from September 2022 to September 2024 were selected. They were divided into the control group and the observation group by random number table method, with 34 cases in each group. The therapeutic effect, blood pressure, blood lipid indexes [serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C)] and cardiac function indexes [left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD)] were compared between the two groups. 【Results】The clinical effective rate of the observation group was 94.12% (32/34), which was higher than 73.53% (25/34) of the control group, and the difference was statistically significant (χ²=5.314, P<0.05). After treatment, the systolic blood pressure and diastolic blood pressure of both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After treatment, the serum levels of TC, TG and LDL-C in both groups were lower than those before treatment, while the HDL-C level was higher than that before treatment; compared with the control group, the observation group had lower serum TC, TG and LDL-C levels and higher HDL-C level, with statistically significant differences (P<0.05). After treatment, the LVEF of both groups was higher than that before treatment, and the LVESD and LVEDD were lower than those before treatment; the observation group had higher LVEF and lower LVESD and LVEDD than the control group, with statistically significant differences (P<0.05). 【Conclusion】 Amlodipine combined with atorvastatin calcium tablets can effectively improve blood pressure, blood lipids and cardiac function in elderly patients with hypertension complicated by coronary heart disease, and has good clinical application value.
