Abstract 【Objective】 To explore the application effect of percutaneous minimally invasive pedicle screw internal fixation combined with posterior decompression and bone grafting in elderly patients with spinal fractures. 【Methods】 A total of 108 elderly patients with spinal fractures admitted to our hospital from January 2021 to December 2024 were selected and divided into the control group (treated with percutaneous minimally invasive pedicle screw internal fixation) and the observation group (treated with percutaneous minimally invasive pedicle screw internal fixation combined with posterior decompression and bone grafting), with 54 cases in each group. The surgical-related indicators, spinal function [Oswestry Disability Index (ODI) score], pain degree [Visual Analogue Scale (VAS) score], and vertebral body recovery were compared between the two groups. 【Results】 The intraoperative blood loss in the observation group was greater than that in the control group, and the operation time and incision healing time were longer than those in the control group (P<0.05). At 3 months after surgery, there was no statistically significant difference in the VAS score between the two groups (P>0.05). The ODI score in the observation group was lower than that in the control group (P<0.05), the posterior convex Cobb angle of the vertebral body in the observation group was smaller than that in the control group (P<0.05), and the anterior and middle height of the vertebral body were higher than those in the control group (P<0.05). 【Conclusion】 Although percutaneous minimally invasive pedicle screw internal fixation combined with posterior decompression and bone grafting prolongs the operation time and incision healing time and increases blood loss, it can more effectively improve the spinal function of patients with spinal fractures, correct vertebral body deformity, and maintain vertebral body height.
