Correlation of Postoperative Prognosis in Patients with Esophageal Cancer with 18F-FDG PET-CT Metabolic Parameters and Serum Levels ALB and PA

Abstract  【Objective】To explore the correlation of postoperative prognosis in patients with esophageal cancer with fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)-CT metabolic parameters and levels of serum albumin (ALB) and prealbumin (PA).【Methods】Ninety patients with esophageal cancer were divided into two groups according to prognosis: 30 patients with good prognosis and 60 patients with poor prognosis. General information,18F-FDG PET-CT metabolic parameters,serum nutritional indicators (ALB and PA) of both groups were collected and compared. Receiver operating characteristic curve (ROC) analysis was used to evaluate the value of 18F-FDG PET-CT metabolic parameters,ALB,and PA in predicting postoperative prognosis in patients with esophageal cancer,and multiple logistic regression analysis was used to identify the risk factors for poor postoperative prognosis.【Results】There were no significant differences in age,sex,smoking history,drinking history,height,weight,and family history between the two groups (P>0.05). The maximum standard uptake value (SUVmax),metabolic volume (MTV),and total lesion glycolysis (TLG) in the poor prognosis group were higher than those in the good prognosis group (P<0.05). The levels of ALB and PA in the poor prognosis group were lower than those in the good prognosis group (P<0.05). ROC analysis showed that SUVmax,MTV,TLG,ALB,PA could be used to predict poor postoperative prognosis in patients with esophageal cancer,with the areas under the curve being 0.985,0.913,0.963,0.726 and 0.731 respectively (all P<0.05). Multivariate logistic regression analysis confirmed that SUVmax ≥ 21.25,MTV ≥ 14.42 cm3,TLG ≥ 5.85,ALB < 52.795 g/L,and PA < 230.09 mg/L were risk factors for poor postoperative prognosis in patients with esophageal cancer (P<0.05).【Conclusion】The 18F-FDG PET-CT metabolic parameters and serum levels of ALB and PA can be used for predicting the prognosis of surgical patients with esophageal cancer. Clinicians can regularly monitor these indicators and promptly carry out postoperative follow-up chemotherapy to improve the prognosis.
