Expression and Significance of Serum VEGF and ET-1 in Patients with Refractory Diabetic Foot |

Abstract 【Objective】To investigate the expression and significance of serum vascular endothelial growth factor (VEGF) and endothelin-1 (ET-1) in patients with refractory diabetic foot. 【Methods】A total of 101 patients with diabetic foot admitted to our hospital from July 2019 to November 2021 were selected as the study group,while 68 patients with simple diabetes (without diabetic foot) admitted to our hospital during the same period were selected as the control group. The basic data of patients in the two groups were collected to analyze the factors affecting the occurrence of diabetic foot and the predictive value of serum VEGF and ET-1 in diabetic foot. The levels of serum VEGF and ET-1 in patients with different grades of diabetic foot in the study group were statistically analyzed. 【Results】The levels of fasting blood glucose (FPG),glycosylated hemoglobin,vascular cell adhesion molecule-1 (VCAM-1),fibroblast growth factor 2 (FGF2),C-reactive protein (CRP) and ET-1 in the study group were significantly higher than those in the control group (P<0.05). The level of VEGF was significantly lower than that in the control group (P<0.05). Logistic multifactor regression analysis showed fasting blood glucose (FPG),vascular cell adhesion molecule-1 (VCAM-1),VEGF and ET-1 were risk factors affecting the occurrence of diabetic foot (P<0.05). ROC curve results showed that the sensitivity of serum VEGF,ET-1 and their combination in predicting diabetic foot occurrence was 70.30%,78.22% and 70.30%,respectively; their specificities were 70.59%,79.41% and 94.12%,respectively and their AUC values were 0.757,0.766,and 0.910,respectively. There were statistically significant differences of VEGF and ET-1 in grade Ⅰ to Ⅴdiabetic foot groups (P<0.05). The levels of ET-1 in grade Ⅰ to Ⅴ diabetic foot groups were significantly increased with the increase of grading (P<0.05);While VEGF levels in grade Ⅰ to Ⅴ diabetic foot groups were significantly decreased with the increase of grading (P<0.05). 【Conclusion】Serum VEGF and ET-1 are not only related to the occurrence of diabetic foot,but also related to its progression to refractory diabetic foot. VEGF combined with ET-1 has a better early prediction of diabetic foot occurrence.
