腹腔镜子宫肌瘤剔除术后患者的妊娠结局及其影响因素

Abstract 【Objective】To investigate the pregnancy outcome and its influencing factors of patients after laparoscopic myomectomy. 【Methods】 A total of 216 patients with fertility requirements who underwent laparoscopic myomectomy in our hospital from August 2018 to August 2020 were selected, and were divided into the observation group (single leiomyoma, n=142) and the control group (multiple leiomyomas, n=74) according to the number of leiomyomas. The pregnancy, delivery and abortion of the two groups were compared, and the independent risk factors of adverse pregnancy outcomes were analyzed by multivariate logistic regression. 【Results】 There was no significant difference in pregnancy rate, live birth rate, ectopic pregnancy rate and abortion rate between the two groups (P>0.05). 33 of 216 patients (15.28%) had adverse pregnancy outcomes. Multivariate logistic regression analysis showed that age, large number of myomas, and preoperative intrauterine adhesions were independent risk factors for adverse pregnancy outcomes after laparoscopic myomectomy. 【Conclusion】 Age, the number of myomas and intrauterine adhesions before operation are all important factors that affect the pregnancy outcome of patients after laparoscopic myomectomy. Clinical attention should be paid to these factors, and preventive treatment should be carried out to improve the pregnancy outcome.
