Effect of Immediate Implantation with Modified Shield Tunneling on the Bone Mass, Periodontal Tissue and Aesthetic Effect of the Upper Anterior Teeth

Abstract  【Objective】 To investigate the effect of modified shield tunneling (modified SST) immediate implantation on the bone mass, periodontal tissue and aesthetic effect of the labial maxillary anterior teeth. 【Methods】 From November 2016 to November 2021, 120 patients with immediate implant repair of a single tooth in the maxillary anterior tooth area who were treated in Ankang Hospital of Traditional Chinese Medicine were selected as the study objects, of which 60 patients were treated with traditional immediate implant as the control group, and 60 patients were treated with modified SST as the observation group. After 12 months of follow-up, the implant success rate, bone absorption at the lip side of the implant, and periodontal tissue around the implant were compared between the two groups [Including modified gingival sulcus bleeding index, periodontal probing depth, gingival papilla index, red aesthetic index (PES)].The incidence of adverse reactions of all patients was counted.【Results】The successful rate of implantation was 100% in both groups. The improved gingival sulcus bleeding index in the observation group was significantly lower than that in the control group (P<0.05). At 6 months and 12 months after repair, there was no significant difference in the improved gingival sulcus bleeding index between the two groups (P>0.05); The gingival papilla index of the observation group was higher than that of the control group at 6 months and 12 months after repair (P<0.05). Lip bone resorption in the observation group was significantly lower than that in the control group 6 months after repair and 12 months after repair (P<0.05). Lip bone absorption was higher in both groups 12 months after repair than in 6 months after repair. The bone resorption at the lip side of the patients in the observation group was lower than that in the control group 6 months and 12 months after the repair (P<0.05). The PES scores of the two groups increased 6 and 12 months after repair, and the scores of the observation group were higher than those of the control group (P<0.05). There was no significant difference in the incidence of adverse reaction between the two groups (P>0.05). 【Conclusion】 The modified SST immediate implantation has a positive effect on maintaining the bone mass of the lip, and can obtain better soft tissue aesthetic effect with high safety.
