Study on Serum ALD,AngII Levels and their Correlation in Patients with Acute Myocardial Infarction Complicated with Heart Failure |

Abstract 【Objective】 To investigate the levels of serum aldosterone (ALD) and angiotensin Ⅱ(AngⅡ) in patients with acute myocardial infarction (AMI) and heart failure (HF) and their correlation. 【Methods】 A total of 280 AMI patients admitted to our hospital from November 2017 to October 2020 were selected. According to whether AMI patients had HF,280 patients were divided into non HF group (n=219) and HF group (n=61). Serum N-terminal pro-B-type natriuretic peptide (NT-proBNP),renin (REN),ALD and AngⅡwere detected by enzyme-linked immunosorbent assay. Multivariate logistic regression model was used to analyze the risk factors of HF in AMI patients. ROC curve was used to analyze the predictive value of serum NT-proBNP,REN,ALD and AngⅡ levels for HF in AMI patients.【Results】 LVEF in the HF group was significantly lower than that in the non HF group,and serum NT-proBNP level was significantly higher than that in the non HF group (P<0.05). The levels of serum REN,ALD and AngⅡ in the HF group were significantly higher than those in the non HF group (P<0.05). The expression levels of REN,ALD and AngⅡ in serum of patients with different Killip grades were significantly different (P<0.05); The serum levels of REN,ALD and AngⅡ in Killip Ⅲ and Ⅳ patients were higher than those in KillipⅡ patients,and those in Killip Ⅳ patients were higher than those in Killip Ⅲ patients,the difference was statistically significant (P<0.05). Multivariate logistic analysis showed that the levels of NT proBNP,REN,ALD and AngⅡ in serum were independent risk factors for HF in patients with AMI (P<0.05). ROC curve analysis showed that AUC predicted by serum ALD level was higher than NT-proBNP,REN and AngⅡ (P<0.05),and AUC predicted by AngⅡ and REN levels was higher than NT-proBNP (P<0.05). 【Conclusion】Serum ALD and AngⅡ are highly expressed in patients with AMI complicated with HF,which can be used as indicators to predict the occurrence of HF,and the level of serum ALD has a higher predictive value.
