| Clinical Efficacy and Safety of Flexible Ureteroscopic Lithotripsy with Electronic Ureteroscope Versus Percutaneous Nephrolithotomy in the Treatment of Large Renal Pelvic Stones |

Abstract 【Objective】 To explore the clinical efficacy and safety of flexible ureteroscopic lithotripsy with electronic ureteroscope (f-URL) versus percutaneous nephrolithotomy (PCNL) in the treatment of large renal pelvic stones. 【Methods】 A total of 102 patients with large renal pelvic stones (long diameter > 3.0 cm) admitted to our hospital from March 2022 to March 2024 were selected. They were divided into a control group and an observation group using a random number table, with 51 cases in each group. The control group was treated with PCNL, while the observation group was treated with f-URL. Perioperative indicators (intraoperative blood loss, operation time, average length of hospital stay), incidence of adverse reactions, stone clearance rate, and proportion of conversion to open surgery during operation were compared between the two groups. 【Results】 The operation time of the observation group was longer than that of the control group, while the intraoperative blood loss was less and the average length of hospital stay was shorter than those of the control group, with statistically significant differences (P<0.05). There were no statistically significant differences in stone clearance rate or the proportion of conversion to open surgery between the two groups (P>0.05). The incidence of postoperative complications in the observation group was 5.88% (3/51), which was lower than 23.53% (12/51) in the control group, with a statistically significant difference (P<0.05). 【Conclusion】 Both f-URL and PCNL are effective surgical methods for treating large renal pelvic stones (long diameter > 3.0 cm). Compared with PCNL, f-URL has obvious advantages in terms of intraoperative blood loss, length of hospital stay, and incidence of adverse reactions.
