| Impact of Clinical Prognosis-Oriented Home Transitional Care Program on Cardiac Function and Self-Management Ability in Patients after PCI |

Abstract 【Objective】 To explore the impact of a clinical prognosis-oriented home transitional care program on cardiac function and self-management ability in patients after percutaneous coronary intervention(PCI). 【Methods】 A total of 881 patients who underwent emergency PCI in our hospital from January 2022 to April 2024 were selected. The hospital launched the clinical prognosis-oriented nursing intervention in March 2023. According to the admission time, the patients were divided into a control group(January 2022 to February 2023, n=417) and an observation group(March 2023 to April 2024, n=464). The control group received routine postoperative nursing after PCI, while the observation group received the clinical prognosis-oriented home transitional care program. The postoperative cardiac function [6-minute walking distance(6MWD), left ventricular ejection fraction(LVEF), left ventricular end-diastolic diameter(LVEDD)], self-management ability, and the incidence of major adverse cardiovascular events(MACE) were compared between the two groups. 【Results】 At 3 months after discharge, the 6MWD and LVEF of both groups were higher than those on the 1st day before discharge, and the LVEDD was lower than that on the 1st day before discharge. Moreover, the 6MWD and LVEF of the observation group were higher than those of the control group, and the LVEDD was lower than that of the control group, with statistically significant differences(P<0.05). At 3 months after discharge, the scores of daily life management, disease knowledge management, treatment compliance, emotional management, first-aid skills and the total score of both groups were higher than those on the 1st day before discharge, and the scores of the observation group were higher than those of the control group, with statistically significant differences(P<0.05). The incidence rates of postoperative revascularization, ventricular arrhythmia, post-infarction angina pectoris and in-stent restenosis in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). 【Conclusion】 The clinical prognosis-oriented home transitional care program can significantly improve postoperative cardiac function, enhance self-management ability, and reduce the risk of MACE after PCI.
