Influence of Modified Delorme Procedure Combined with PPH on Anal Function and Serum Stress Indexes in Patients with Rectal Prolapse |

Abstract 【Objective】To investigate the influence of modified rectal mucosal sleeve resection and intestinal wall plication(Delorme procedure) combined with procedure for prolapse and hemorrhoids (PPH) on anal function and serum stress indexes in patients with rectal prolapse.【Methods】 A retrospective analysis was performed on the clinical data of 60 patients with rectal prolapse admitted to our hospital from March 2023 to March 2024. The patients were divided into the observation group and the control group according to treatment methods, with 30 cases in each group. The control group received the modified Delorme procedure, while the observation group received the modified Delorme procedure combined with PPH. The curative effect, complications, rectal pressure, serum stress indexes [prostaglandin E2 (PGE2), cortisol (COR), norepinephrine (NE)], and anal function recovery were compared between the two groups at 3 months postoperatively. 【Results】The total effective rate of the observation group was higher than that of the control group (P<0.05). At 3 months postoperatively, the scores of anal distension and fullness, fecal continence ability, and anal moisture in both groups were significantly lower than those before surgery, and the observation group had lower scores than the control group (P<0.05). The maximum resting pressure and maximum systolic pressure in both groups were higher than those before surgery, while the rectal sensory volume was lower than that before surgery. The observation group had higher maximum resting pressure and maximum systolic pressure, and lower rectal sensory volume than the control group (P<0.05). The serum levels of PGE2, COR, and NE in both groups were higher than those before surgery (P<0.05), but there was no significant difference between the two groups (P>0.05). The complication rate in the observation group was lower than that in the control group (P<0.05). 【Conclusion】The modified Delorme procedure combined with PPH has a good clinical effect in the treatment of rectal prolapse. It can regulate rectal pressure, improve anal distension and fullness fecal continence ability, and anal moisture, and reduce the incidence of postoperative complications.
