The Value of Serum 25-Hydroxyvitamin D Combined with Clinical Data in the Prognostic Assessment of Patients with Cerebral Ischemic Stroke

Abstract  【Objective】To explore the application value of serum 25-hydroxyvitamin D [25(OH)D] combined with clinical data in the prognostic assessment of patients with Cerebral Ischemic Stroke (CIS). 【Methods】 Clinical data of 1000 CIS patients treated in our hospital from January 2018 to December 2019 were collected. Based on their prognosis, patients were divided into the good prognosis group (survival time ≥ 3 months, n=510) and the poor prognosis group (survival time < 3 months, n=490). The basic data of the two groups were compared. The value of 25(OH)D level in predicting poor prognosis in CIS patients was analyzed using the Receiver Operating Characteristic (ROC) curve. The risk factors for poor prognosis in CIS patients were identified through a Logistic regression analysis model. 【Results】 The proportion of patients with a Body Mass Index (BMI) > 23.99 kg/m2, onset to hospital admission time > 24 h, smoking history, infarct area ≥ 1.5 cm2, hyperglycemia, high body temperature, National Institutes of Health Stroke Scale (NIHSS) score ≥ 21, and Combined with hypertension was higher in the poor prognosis group than in the good prognosis group (P<0.05). Serum 25(OH)D levels were lower in the poor prognosis group (P<0.05). ROC analysis showed that the area under the curve (AUC) of 25(OH)D for predicting poor prognosis was 0.680, with 95%CI of 0.586-0.775, the sensitivity of 0.450, and the specificity of 0.700 (P<0.05). Multifactorial Logistic results indicated that BMI > 23.99 kg/m2, onset to admission time > 24 h, smoking history, infarct area ≥ 1.5 cm2, hyperglycemia, NIHSS score ≥ 21, high body temperature, hypertension, and 25(OH)D < 24.500 ng/mL were risk factors for poor prognosis (P<0.05). 【Conclusion】 The poor prognosis of CIS patients is closely related to BMI, onset to admission time, smoking history, infarct area, hyperglycemia, high body temperature, hypertension, and 25(OH)D level. The level of 25(OH)D can predict the prognosis of this disease.
