Effect of Simultaneous Heart Valve Replacement and Coronary Artery Bypass Grafting on Cardiac Function in Patients with Moderate Coronary Artery Disease and Heart Valve Disease

Abstract  【Objective】 To investigate the effect of simultaneous heart valve replacement and coronary artery bypass grafting on cardiac function in patients with moderate coronary artery disease and heart valve disease. 【Methods】The clinical data of 114 patients with moderate coronary artery disease and heart valve disease admitted to our hospital from January 2019 to June 2020 were retrospectively analyzed. According to the different treatment schemes, they were divided into observation group and control group, with 57 patients in each group. The control group received non simultaneous heart valve replacement + coronary artery bypass grafting, and the observation group received simultaneous heart valve replacement + coronary artery bypass grafting. The related indexes of the two groups, the incidence of major cardiovascular adverse events at 6 months after the operation, and the cardiac function [left ventricular ejection fraction (LVEF), left ventricular end diastolic volume, cardiothoracic ratio] at 6 months before and after the operation were compared, The scores of Seattle Angina Questionnaire (SAQ) were compared before and after treatment.【Results】The aortic occlusion time in the observation group was longer than that in the control group, and the ventilator circulation time, the hospitalization days in the general ward, and the ICU monitoring time in the cardiac surgery department were shorter than that in the control group (P<0.05). At 6 months after operation, the cardiothoracic ratio and left ventricular end diastolic volume in the observation group were lower than those in the control group, and the LVEF was higher than that in the control group (P<0.05). The incidence of malignant arrhythmia, acute myocardial infarction, intractable myocardial ischemia, atrial fibrillation and acute heart failure in the observation group was lower than that in the control group (P<0.05). At 3 and 6 months after operation, the SAQ score of the observation group was higher than that of the control group (P<0.05). 【Conclusion】Simultaneous cardiac valve replacement and coronary artery bypass grafting can improve cardiac function, reduce the incidence of major cardiovascular adverse events and improve the quality of life of patients with moderate coronary artery disease and heart valve disease.
