Abstract  【Objective】To explore the serum 25-hydroxyvitamin D level and its significance in patients with autoimmune disease and vitiligo. 【Methods】A total of 67 patients with autoimmune disease and vitiligo who were admitted to the hospital (the observation group). While 54 healthy people without autoimmune disease and vitiligo who received physical examination at the same time period were selected as the control group. The basic data of the two groups were collected. Clinical characteristics and serum 25-hydroxyvitamin D levels of the two groups were compared and the factors affecting the occurrence of autoimmune disease and vitiligo were analyzed. The area under the receiver operating characteristic (ROC) curve (AUC) was used to evaluate the value of serum 25-hydroxyvitamin D in diagnosing the occurrence of autoimmune disease and vitiligo. 【Results】There were no statistically significant differences in gender, age, body mass index, comorbidities, smoking history, drinking history and history of sun exposure between the groups (P>0.05). The proportion of blood group A, family history of vitiligo, CD14+ T lymphocytes, CD4+ T lymphocytes, CD8+ T lymphocytes, interleukin-6, interleukin-4, interleukin Su-21 level in the observation group was higher than control group, the difference was statistically significant (P<0.05). The level of 25-hydroxyvitamin D in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Logistic multivariate analysis showed that 25-hydroxyvitamin D and CD8+ T lymphocytes were the influencing factors of vitiligo (P<0.05). The sensitivity, specificity and AUC of serum 25-hydroxyvitamin D in the diagnosis of vitiligo were 77.61%, 74.07%, and 0.786, respectively. 【Conclusion】The serum 25-hydroxyvitamin D level of patients with autoimmune disease and vitiligo is lower than healthy people. So the 25-hydroxyvitamin D may have a certain relationship with the occurrence of autoimmune disease vitiligo.
