Relationship of TLR4/NF-κB Pathway with Related Cytokine Expression and Development of Acute Pancreatitis

Abstract  【Objective】To explore the relationship of Toll-like receptor 4 (TLR4)/nuclear factor kappa B (NF-κB) pathway with related cytokine expression levels and the development of acute pancreatitis (AP). 【Methods】A total of 127 AP patients admitted to our hospital were divided into the AP group (75 cases) and the severe acute pancreatitis (SAP) group (52 cases) based on the severity of the illness. And 80 healthy volunteers who underwent physical examination in our hospital during the same time period were selected as the control group. The clinical data,TLR4,NF-κB expression levels and serum inflammatory factor levels were compared among the three groups. Pearson correlation was used to analyze the relationship of TLR4/NF-κB pathway with related cytokine expression levels and acute pancreatitis bedside severity index (BISAP). 【Results】The serum lipase and amylase of the AP group and SAP group were significantly higher than those of the control group (P<0.05). TLR4 mRNA and NF-κB mRNA in the AP group and the SAP group were higher than those in the control group (P<0.05). TLR4 mRNA and NF-κB mRNA in the SAP group were significantly higher than those in the AP group (P<0.05). The levels of interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the AP group and SAP group were significantly higher than those in the control group (P<0.05). The levels of IL-6,TNF-α and IL-1β in the SAP group were significantly higher than those in the AP group (P<0.05). The levels of TLR4 mRNA,NF-κB mRNA,TNF-α and IL-1β were all positively correlated with BISAP (P<0.05),while there was no correlation between IL-6 levels and BISAP (P>0.05). 【Conclusion】The changes in TLR4/NF-κB pathway and related cytokine expression levels have a certain correlation with the development of AP disease,which can be a reference for clinical evaluation of AP disease progression.
