Application of Medical Failure Mode and Effect Analysis in Process Management of Indwelling Catheter in Patients with Benign Prostatic Hyperplasia

Abstract  【Objective】To explore the application effect of failure mode and effect analysis in the process management of indwelling urinary catheter in patients with benign prostatic hyperplasia (BPH). 【Methods】 A total of 33 patients with BPH indwelling catheter admitted to the geriatric department of our hospital from January to June 2019 (before the implementation of HFMEA Management) were selected as the control group, and 35 patients with BPH indwelling catheter admitted from July to December 2019 (after the implementation of HFMEA Management) were selected as the observation group. HFMEA was used to analyze the indwelling catheter process of BPH patients. The potential high-risk factors in the process were found by calculating the risk priority value (RPN), and the improvement plan was formulated and implemented for the key links with RPN > 125. The nursing effect, satisfaction and success rate of the first catheterization were compared between the two groups.【Results】The success rate of the first catheterization in the observation group was 97.14% (34/35), which was significantly higher than 75.76% (25/33) in the control group (χ2=6.764,P=0.0093<0.05). The failure modes of the observation group, such as insufficient evaluation of the patient's condition, improper manipulation of catheterization, inability to correctly judge whether the catheter is in the bladder, insufficient preparation of patients, improper selection of catheter model and total score, were significantly lower than those of the control group, and the differences were statistically significant (mean P<0.05). The total scores of psychological intervention, pain care, operation technology and satisfaction in the observation group were significantly higher than those in the control group (P<0.05). 【Conclusion】The application of HFMEA management is helpful to improve the process of indwelling catheter in patients with BPH, improve the success rate of indwelling catheter, and improve patient satisfaction, which is worthy of clinical application.
