Abstract 【Objective】 To investigate the pathogen distribution and drug-resistance characteristics in patients with type 2 diabetes mellitus (T2DM) complicated by urinary tract infection. 【Methods】 A total of 120 T2DM patients complicated with urinary tract infection admitted to Jiyuan People's Hospital from January 2022 to December 2025 were enrolled. The patients were divided into group A (asymptomatic urinary tract infection) and group B (symptomatic urinary tract infection), with 60 cases in each group. Urine routine tests were performed for all patients. The two groups were compared in terms of age, gender, blood-glucose control and urine-culture results. The distribution characteristics of pathogens were analyzed, and the drug resistance of major pathogens was compared between the two groups. 【Results】 Statistically significant differences were found between the two groups in age, gender composition, fasting blood-glucose, glycosylated hemoglobin (HbA1c), blood urea nitrogen (BUN) and serum creatinine (Scr) (all P<0.05). A total of 48 strains of pathogens were isolated in group A and 59 strains in group B. Gram-negative bacteria were predominant in both groups: 34 Gram-negative strains (including 14 strains of Escherichia coli) in group A, and 41 Gram-negative strains (including 29 strains of Escherichia coli) in group B. Fourteen and eighteen Gram-positive strains were isolated from group A and group B respectively, among which 5 strains and 16 strains were Enterococcus faecalis. Drug-resistance analysis showed that Escherichia coli from both groups presented high resistance to ampicillin, piperacillin and cotrimoxazole; group B showed significantly higher resistance to piperacillin and cotrimoxazole than group A (all P<0.05). Enterococcus faecalis in both groups had high resistance to tetracycline, and the resistance rate in group B was higher than that in group A (P<0.05). 【Conclusion】 Both asymptomatic and symptomatic urinary tract infections in T2DM patients are mainly caused by Gram-negative bacteria, predominantly Escherichia coli, which shows high resistance to ampicillin, piperacillin and cotrimoxazole. For Gram-positive bacteria represented by Enterococcus faecalis, high tetracycline resistance is observed.
