Relationship between Brain Structural Changes and Cognitive Function in Subcortical Ischemic Cerebrovascular Disease |

Abstract 【Objective】To investigate the relationship between brain structural changes and cognitive function in patients with subcortical ischemic vascular disease (SIVD).【Methods】 A total of 134 SIVD patients were divided into a non-cognitive impairment group (MoCA scores≥26, 38 cases, Group A) and a cognitive impairment group (MoCA scores<26, 96 cases, Group B) based on the Montreal Cognitive Assessment (MoCA) scores. The brain structural changes and cognitive functions between the two groups were compared, including differences in cerebrospinal fluid volume, white matter volume, and gray matter volume. Brain regions with significant differences in gray matter volume were compared, and the correlation between the gray matter volume of these regions and cognitive function scores in Group B was analyzed.【Results】 The Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) scores in Group B were higher than those in Group A (P<0.05). The Rey Auditory Verbal Learning Test (RAVLT) delayed recall and immediate recall scores, MoCA scores, Mini-Mental State Examination (MMSE) scores, and Verbal Fluency Test (VFT) scores in Group B were lower than those in Group A (P<0.05). The gray matter volume of the left medial superior frontal gyrus and other regions in Group B was lower than that in Group A (P<0.05). In Group B, the gray matter volume of the left insula was negatively correlated with the time taken in part A of the Stroop Color-Word Test (CWT-A) (P<0.05); the gray matter volume of the left anterior insula was positively correlated with MMSE scores and Symbol Digit Modalities Test (SDMT) scores (P<0.05); the gray matter volume of the left anterior cingulate gyrus was positively correlated with MMSE, VFT, and SDMT scores (P<0.05).【Conclusion】 Patients with SIVD and cognitive impairment exhibits gray matter atrophy in certain brain regions. The gray matter volume of the left insula, left anterior insula, and left anterior cingulate gyrus is mainly associated with language, attention, cognition, and executive function.
