Evaluation of Placental Blood Flow Parameters in Patients with Gestational Diabetes Mellitus in the Third Trimester by Three-dimensional Ultrasound |

Abstract 【Objective】To explore the value of three-dimensional ultrasound in evaluating placental blood flow parameters in patients with gestational diabetes mellitus(GDM) in the third trimester. 【Methods】 The clinical data of 100 pregnant women with GDM in the third trimester admitted to our hospital from January to December 2021 were retrospectively analyzed, and they were set as the observation group; In addition, 100 normal pregnant women who underwent regular prenatal examination in our hospital during the same period were selected as the control group. Three dimensional Doppler ultrasound was used to examine the placental vascular status and observe the fetal intrauterine development in both groups. The placental blood flow parameters [vascularization index(VI), blood flow index(FI), blood flow/vascular ratio(VFI)] were compared between the two groups; Spearman rank correlation was used to analyze the correlation between blood glucose and placental blood flow parameters in the third trimester of GDM patients; The neonatal condition and Apgar score were compared between the two groups; The pregnancy outcomes of the two groups were compared. 【Results】The levels of VI, FI and VFI in the observation group were lower than those in the control group(P<0.05); The correlation analysis showed that the blood glucose index in the third trimester of GDM patients was negatively correlated with VI, FI and VFI(P<0.05); The proportion of fetal distress, macrosomia and intrauterine growth restriction(FGR) in the observation group was higher than that in the control group(P<0.05), and the Apgar score was lower than that in the control group(P<0.05); The proportion of adverse pregnancy outcomes in the observation group was higher than that in the control group. 【Conclusion】 Three dimensional ultrasound can accurately reflect the placental blood flow parameters, and the placental blood flow parameters are closely related to GDM. It can be used to predict the pregnancy outcome and perinatal status in clinic, and provide the basis for clinical early development of countermeasures.
