Effect of Pelvic External Fixation through Iliac Screw Passage in the Treatment of Patients with Unstable Pelvic Fractures |

Abstract 【Objective】To investigate the clinical effect of pelvic external fixation through iliac screw passage in the treatment of patients with unstable pelvic fractures. 【Methods】Fifty-two patients with Tile C pelvic fractures who were treated in Tangdu Hospital (affiliated to Air Force Medical University) were included in this study. Patients treated with pelvic external fixator via iliac screw path were included in the observation group (n=32),while those treated with plate fixation in front of sacroiliac joint were included in the control group (n=20). Surgical conditions of the 2 groups were comparatively analyzed. The Matta score at 6 weeks after surgery,and Majeed functional scores at 6 weeks,6 months and 12 months after surgery were evaluated. Adverse events were recorded as well. 【Results】The surgical time of the two groups was similar (P>0.05). The intraoperative blood loss and hospital stay of the observation group were less/shorter than those of the control group (P<0.05). At 6 weeks after surgery,the excellent and good rate of Matta score in the observation group was significantly higher than that in the control group (P<0.05). At 6 weeks after surgery,the excellent and good rate of Majeed functional scores in the observation group was significantly higher than that in the control group (P<0.05),but there was no significant difference between the groups at 6 months and 12 months after surgery (P>0.05). Follow-up of 12 months found that there was no pelvic ring deformity,loose screw or nonunion in either group. There was no significant difference in the incidence of adverse events between the groups (P>0.05). 【Conclusion】The pelvic external fixator via iliac screw path can effectively improve fracture displacement in patients with unstable pelvic fractures,which is conductive to pelvic function recovery with few adverse events.
