外周血循环肿瘤细胞阴性富集联合FISH技术诊断乳腺癌的应用价值

Abstract 【Objective】To explore the application of negative enrichment of circulating tumor cells (CTC) in peripheral blood combined with fluorescence in situ hybridization (FISH) in diagnosis of breast cancer.【Methods】A total of 94 suspected breast cancer patients were underwent CTC negative enrichment test combined with FISH. The diagnostic value of negative enrichment of peripheral blood circulation tumor cells combined with FISH technique was analyzed. According to the gold standard of pathological results from puncture biopsy,94 patients were divided into the observation group (with breast cancer group) and the control group (non-breast cancer group).The clinical characteristics of patients with different CTC expression were recorded and the 2-year survival rates of patients with different CTC expression were compared.【Results】Of the 94 patients,there were 78 patients with breast cancer and 16 with benign breast lesions. The number of CTC cells in the observation group was (4.85±1.93) and the control group was (1.57±0.62);There was significant difference between the two groups (P<0.05).The ROC analysis showed that the AUC of CTC number for breast cancer was 0.706,whose sensitivity was 0.615 and specificity was 0.875. Among the 78 cases of breast cancer,58 cases were CTC positive and 20 cases were CTC negative. There were no significant differences in CTC positive detection rates among patients of different ages,body mass index,T stage and N stage (P>0.05).However,there were significant differences in the positive rates of CTC in patients with different M stage and differentiation degree (P<0.05).The 2-year progression-free survival rate of CTC negative patients was 80.00%,which was significantly higher than that of CTC positive patients of 55.17%,the difference was statistically significant. (P<0.05). The 2-year overall survival rates of CTC positive and negative patients were 70.69% and 80.00%,respectively,there was no significant difference between the two groups(P>0.05).【Conclusion】The CTC enrichment combined with FISH is helpful to improve the diagnostic specificity of breast cancer. The CTC is related to the stage of breast cancer. So it is helpful to evaluate the prognosis of patients by detecting the expression of CTC.
