Abstract:【Objective】 To explore the application effect of transitional care based on the Triangle hierarchical management model for chronic diseases in patients with diabetic foot. 【Methods】 A total of 98 patients with diabetic foot admitted to our hospital from January 2022 to February 2024 were selected and divided into an observation group(transitional care based on the Triangle hierarchical management model for chronic diseases) and a control group(routine care) using the random number table method, with 49 cases in each group. The scores of the Diabetes Foot Self-Care Questionnaire-University of Malaga(DFSQ-UMA), Summary of Diabetes Self-Care Activities(SDSCA), Diabetes Specific Quality of Life Scale(DSQL), and the incidence of diabetic foot ulcers were compared between the two groups before intervention and 3 months after intervention. 【Results】 Three months after intervention, the scores of each dimension of DFSQ-UMA and SDSCA in the observation group were significantly higher than those in the control group(P<0.05), while the scores of each dimension of DSQL and the incidence of foot ulcers were significantly lower than those in the control group(P<0.05). 【Conclusion】 Transitional care based on the Triangle hierarchical management model for chronic diseases can significantly improve the self-care ability, self-management behavior and quality of life of patients with diabetic foot, and reduce the incidence of diabetic foot ulcers, thus effectively improving the therapeutic effect and quality of life of patients with diabetic foot.
